The following is an itemized list of violations that have not been addressed for MARCH <br />LANE 76* as of February 27, 2026. <br />Open violations from December 18, 2025 inspection <br />OBSERVATION: Financial responsibility documents have not been submitted to the CUPA . Current financial <br />responsibility documents are required to be submitted annually. <br />CORRECTIVE ACTION: Complete and submit a copy of the financial responsibility by 1/17/2026.. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Open violations from December 12, 2024 inspection <br />Violation #24 - 2030003 - Leak detection system maintains continuous monitoring that activates an audible <br />and visual alarm. <br />OBSERVATION: Floats and chains failed to stop the flow of product at the dispenser when tested. <br />-91 grade on the 1/2 dispenser <br />-91 grade on the 3/4 dispenser <br />-91 grade on the 9/10 dispenser <br />-D grade on the 9/10 dispenser <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to activate an audible and visual alarm or <br />stop the flow of product at the dispenser when it detects a leak. <br />CORRECTIVE ACTION: The service technician adjusted the float and chain and verified functionality. <br />_____________________ <br />OBSERVATION: The 87 grade float and chain in the 9/10 dispenser failed to stop the flow of product at the dispenser <br />when tested. <br />REGULATION GUIDANCE: All monitoring equipment shall be maintained to stop the flow of product at the dispenser <br />when it detects a leak. <br />CORRECTIVE ACTION: Correct immediately by having a properly licensed, trained, and certified contractor replace <br />the failed component with a functional component and obtain a permit within one business day from the EHD, if <br />needed. If the failed component can’t be replaced immediately, there is a possibility that the diesel UST system may <br />be red tagged to prevent fuel inputs. <br /> <br />Describe actions taken or will be taken to correct violation: <br />¨ This violation will be corrected by (date): ¨ This violation was corrected <br />¨ Supporting documents included <br />Page 1 of 2
