•�®�► Job Safety Analysis <br /> Drilling and Well Destruction <br /> JOB SAFETY ANALYSIS JOB TITLE: Drilling and Well Destruction <br /> PREPARED BY: Anne Ritcey DATE: January 22, 2019 <br /> JOB STEPS POTENTIAL HAZARDS RECOMMENDED CONTROLS <br /> Driving to/from site Vehicle collisions All drivers must be licensed for the vehicles they <br /> operate. Always wear seat belts. Drive defensively <br /> and follow traffic laws. The use of cell phones or <br /> other two-way communication devices while driving <br /> is prohibited. <br /> Decontamination Exposure to contamination Wear adequate PPE, such chemical-resistant <br /> gloves, Tyvek, respirators (if needed) and eye <br /> protection. Follow HSP-1 (PPE) and HSP-2 <br /> (Respiratory Protection) as required. <br /> General hazards Slips, trips, and falls Wear skid-resistant soles and steel-toe boots. Be <br /> aware of surroundings, including unstable ground <br /> and construction debris. Take extra precautions <br /> when wearing PPE. <br /> Biological Remain alert to the presence of insects and animals. <br /> If bitten, immediately seek medical attention. <br /> Electrical hazards Inspect all electrical equipment and power cords <br /> prior to use. Don't use damaged cords or equipment. <br /> Use GFCls when using electric powered equipment. <br /> Weather-related hazards Wear appropriate clothing for weather and slip- <br /> resistant footwear during storm events. Monitor for <br /> heat and cold stress, especially when wearing PPE. <br /> Drilling/Working Underground Utilities Be aware of surroundings. Call utility locator and <br /> below ground maintain adequate space around known utility <br /> locations. <br /> Excessive noise Use hearing protection as needed and when drill rig <br /> is in operation. <br /> Vehicle and equipment CDIM staff will not handle or operate drilling <br /> operation equipment. Be aware of surroundings. Wear high- <br /> visibility vest and hard-hat. Maintain safe distance <br /> from equipment. Watch for moving vehicles. <br /> CDIM 1 <br />
