HSC 117960 – A large quantity generator required to register with the enforcement agency (EA) shall file with the <br />enforcement agency a medical waste management plan, on forms prescribed by the enforcement agency. The plan <br />shall include but not be limited to all elements noted in this section. <br />OBSERVATIONS: <br />1. Facility did not have a medical waste management plan to provide on site for review. <br />CORRECTIVE ACTIONS: <br />1. Provide the EA with a copy of the Medical Waste Management Plan and keep a copy of the Medical Waste <br />Management Plan on site for the EA to review when requested. <br /> <br />13. HSC 117975 – A large quantity medical waste generator required to register pursuant to this chapter shall maintain <br />for a minimum of two years individual treatment records and tracking the tracking document for all untreated medical <br />waste shipped offsite for treatment. <br />OBSERVATIONS: <br />1. Medical waste tracking document dated 1/22/25 did not have proof of destruction /treatment information completed. <br />Online copy of the document was not uploaded. <br />CORRECTIVE ACTIONS: <br />1. Provide to the EA the competed copy of this medical waste tracking document to show proof of destruction/treatment of <br />the medical waste that was picked up on 1/22/25. <br /> <br />23. HSC 117630 (b) – The biohazard bag used to collect medical waste within a facility shall be manufacturer certified to <br />meet the ASTM D1709 dart drop test, provided that when the bag is prepared for transport offsite, it is placed into a <br />USDOT-approved container lined with a biohazard bag that is ASTM D1709 and ASTM D1922 certified. <br />OBSERVATIONS: <br />1. The small biohazard bags and the small chemotherapy waste bags that were provided by the facility for review did not <br />contain the correct certification. <br />CORRECTIVE ACTIONS: <br />1. Supply the facility to ensure that the proper biohazardous bags are being used. Provide proof of supply to EA for review. <br /> <br />24. HSC 118275(a)(4) – Trace Chemotherapy waste shall be segregated for storage, and, when placed in a secondary <br />container, that container shall be labeled with the words “Chemotherapy Waste”, “CHEMO”, or other label approved by <br />the department on the lid and sides, so as to be visible from any lateral direction, to ensure treatment of the <br />biohazardous waste. <br />OBSERVATIONS: <br />1. The chemotherapy waste container in the designated accumulation area did not have the proper labels on all 4 sides of <br />the container. – CORRECTED ON SITE <br /> <br /> <br />
