Underground Storage Tank
Secondary Containment Testing Report Form
9. COMMENTS
Mast=Master. Siph=Siphon. PS=Piping Sump. Western FG=Western Fiberglass.
BP=Bottom Penetrations.
Note: Tank #2 Regular 87 Siphon, and Tank #3 Super 91 Tank Secondary Containment Sensor L3
and Sensor L5 were in alarm when i arrived on site. I found both of the Tank Secondary
Containment Riser Caps loose. When i removed the sensors for testing, the alarms cleared. I
found 3/4" of water in both of the Tank Secondary Containments. I removed approximately 20
ounces of water total from both of the Tank Secondary Containments prior to testing. The water
appeared dirty and sandy, and mirrored the back fill soil material around both of the Tank Secondary
Containment Risers. No smell of fuel present. I applied the Test Vacuum of -10" Hg. for one hour
on both of the Tank Secondary Containments, and no loss of Test Vacuum was detected throughout
the test periods. Possible source of water in both of the Tank Secondary Containments was surface
run off that entered the Tank Secondary Containment through the loose caps on both of the Tank
Secondary Containment Risers. After the Tank Secondary Containment testing was complete, i
placed the sensors back to their proper placement and no further alarms were detected. I applied
Pipe sealant compound to all of the Tank Secondary Containment riser threads to make a proper
seal. I instructed the Owner/Operator to log the alarms in the onsite alarm log with a description of
the alarm event. I advised the Owner/Operator to make sure these Secondary Containment Riser
Caps are secure after all testing events in the future to prevent surface water run off from entering
the Tank Secondary Containment.
All tests marked "Fail'and any repairs made before or during the tightness test must be described in
the COMMENTS section.
Page 5 of 5
