The following is an itemized list of hazardous waste violations that have not been <br />addressed for AM PM HAMMER/15 FOOD #83113 as of ,psi`( 21, ZOZ6 <br />Open violations from July 18, 2022 inspection <br />Violation #110 -Failed to keep signed copy of manifests from the designated facility for three years. <br />OBSERVATION Designated facility signed and dated copies of Uniform Hazardous Waste Manifests (UHWM) <br />were not found on-site. The missing manifests are: <br />- 020390556JJK, ship date 11-14-19 <br />REGULATION GUIDANCE: A hazardous waste generator shall keep a copy of each manifest signed in accordance <br />with section 66262.23(a) for three years or until the generator receives a signed copy from the designated facility <br />which received the waste. This signed copy shall be retained as a record for at least three years from the date the <br />waste was accepted by the initial transporter. The hazardous waste generator must submit a copy of the manifest <br />to the California Department of Toxic Substances Control (DISC) within 60 days if the destination facility signed <br />manifest copy is not received from the transporter or designated facility. <br />CORRECTIVE ACTION: Locate the destination facility signed manifests listed above and maintain in the facility file <br />for three years from the date of shipping. Send a copy to the San Joaquin County Environmental Health <br />Department (EHD). <br />❑ This violation was corrected <br />❑ Supporting documents included <br />❑ This violation will be corrected by (date): <br />Describe actions taken or will be taken to correct violation: <br />Violation #117 -Failed to submit a written response within 30 days of receiving an inspection report. <br />OBSERVATIOI�t This business did not provide a written response to the inspection report dated 5/26/2016 within 30 <br />days. The report was delivered to the business on 5/26/2016. <br />REGULATION GUIDANCE: The operator of the site or facility which receives an inspection report shall submit a <br />written response to the department or the local officer or agency authorized to enforce this chapter pursuant to <br />subdivision (a) of Section 25180 within 60 days of receipt of the inspection report, or within a shorter time as the <br />department or the local officer or agency may reasonably require, which shall include a statement documenting <br />corrective actions taken by the operator or proposing corrective actions which will be taken by the operator, for <br />purposes of compliance with this chapter, or disputing the existence of the violation.
