Other requirements <br /> may also apply. These requirements cannot be modified. <br /> 1) Submit Water Provision Declaration form to the Environmental Health Department for <br /> review. <br /> 2) Applicant shall contact Natalia Subbotnikova, Program Coordinator, Small Public Water System <br /> Program, at (209) 468-0338, to determine if the existing well can be permitted as a public water <br /> system prior to issuance of building permits. If a public water system is required, applicant shall <br /> submit a Small Public Water System preliminary technical report to the California State Water <br /> Resources Control Board, Division of Drinking Water (Water Board) at least six months before <br /> initiating construction of any water related improvement, as defined. The issuance of a permit to <br /> operate a small public water system by the local primacy agency (EHD) is prohibited without the <br /> concurrence of the Water Board. Please contact Gena Farley with the SWRCB Division of Drinking <br /> Water at Gena.Farley@waterboards.ca.gov or 209-948-7488, concerning the requirements for <br /> preliminary technical report submittal prior to issuance of building permits. <br /> If the Water Board determines that an onsite well shall be used as the potable water source, a permit <br /> application to operate Small Public Water System shall be submitted to the EHD for approval prior <br /> to issuance of building permits. To issue a permit to operate, concurrence from the Water Board is <br /> required. A yearly permit to operate a public water system will be required by the EHD prior to sign <br /> off of the certificate of final occupancy (San Joaquin County Development Title, Section 9-602.010 <br /> and 9-601.030.). <br /> The supplier must possess adequate financial, managerial, and technical capability to assure <br /> delivery of pure, wholesome, and potable drinking water in accordance with San Joaquin County <br /> Development Title, Sections 9-602.010 and 9-601.030 and C.C.R., Title 22, and Health and Safety <br /> Code, Section 116525 116570. <br /> 3) Before any hazardous materials/waste can be stored or used onsite, the owner/operator <br /> must report the use or storage of these hazardous materials to the California Environmental <br /> Reporting System (CERS) at cers.calepa.ca.gov/ and comply with the laws and regulations <br /> for the programs listed below (based on quantity of hazardous material in some cases).
