This information was required to be submitted 30 days after receiving the inspection
report. This information has not been received resulting in a non-compliant status for this facility.
The outstanding violations are itemized on the following pages and may include violations from past
inspections. Complete and submit a copy of the Return to Compliance Certification form to the EHD
with a statement documenting the corrective actions that have been or will be taken for each violation,
and any supporting paperwork.
Note: All EHD staff time associated with failing to comply, including the issuance of this
letter, is billed at the current hourly rate ($179). Failure to submit the completed
information immediately may result in a re-inspection, additional violations, and/or further legal action.
If you have any questions, please contact Kristina Saetern at (209) 616-3045.
Thank you,
Kristina Saetern,
Environmental Health Department
