Universal Health & Safety Plan <br /> For use on all high-risk,industrial and HAZWOPER projects <br /> Department of Toxic Substances Control <br /> Table 4-9: Site Emergency Equipment and Its Location <br /> DESCRIPTIONITEM(S) ITEM • • <br /> Wildlife Deterrent(s) ■ N/A ■ N/A <br /> Emergency Shelter(s) ■ N/A ■ N/A <br /> Emergency Ration(s) ■ N/A ■ N/A <br /> Personal Locating Device(s) ■ N/A ■ N/A <br /> Cold/Heat Stress Aids ■ Climate-Controlled Environment ■ Field Vehicle(s) <br /> ■ Other(specify) ■ Location <br /> 4.3 Potential Emergency Scenarios <br /> This section covers emergency scenarios that could reasonably occur on the site or during work. <br /> 4.3.1 Evacuation <br /> ■ If a situation requires an evacuation or emergency muster/assembly,the pre-determined alarm will be initiated. <br /> ■ All personnel (e.g.,workers, contractors, visitors)of the area requiring evacuation or muster/assembly will immediately <br /> assemble at the designated Muster Point,Assembly Point or Shelter-in-Place as determined by the alarm or <br /> communication. <br /> ■ The Site Supervisor, Site Safety Officer, or designate will take action to account for all personnel, including visitors(i.e., <br /> head count, roll call). <br /> ■ The Site Supervisor, Site Safety Officer, or designate shall ensure the appropriate emergency response is activated. <br /> ■ Should it be determined that an individual is still within the hazard zone, establish whether a rescue can be safely <br /> attempted. Follow the'Emergency Rescue Procedure' if properly trained and a rescue attempt will not put another <br /> individual in jeopardy. <br /> ■ Personnel shall await further instruction from the Site Supervisor, Site Safety Officer, or designate (e.g., all clear and re- <br /> entry or further evacuation) <br /> 4.3.2 Medical Emergency <br /> 1. Stop the work activity. <br /> 2. Assess the cause of the injury to avoid injury to yourself(i.e. live wires, gases, hazardous materials). <br /> 3. Do not move the casualty unless they remain in danger. <br /> 4. First Aid Provider will designate an individual to call for medical assistance(e.g., ambulance, site medic). <br /> 5. First Aid Provider will designate an individual to retrieve the first aid kit and blankets. <br /> 6. Request assistance from other First Aid Providers as necessary.Administer first aid: <br /> a) Assess responsiveness: ask permission. <br /> b) Send for medical help. <br /> c) Place casualty/victim face up. <br /> d) Check Airway, Breathing and Circulation ABC's <br /> e) Control severe bleeding. <br /> 7.
