Sample Documentation <br /> 8.1 Field Notes <br /> As applicable, field documentation will include, but not be limited to, the following <br /> • Field notebook detailing dates and times of field work, personnel on-site (including <br /> subcontractors, client representatives, etc.), equipment used, and a summary of the <br /> activities performed <br /> • Photographs, with approval from the site <br /> • Sample COCs <br /> • Health and safety forms (daily tailgate safety meeting, job hazard analysis, etc.) <br /> 8.2 Sample Labeling <br /> A sample label will be affixed to each sample container. The label will be completed with the <br /> following information written in indelible ink: <br /> • Sample identification <br /> • Analysis requested <br /> • Preservative <br /> • Sample collection date <br /> • Sample collection time <br /> • Identification of the sampler <br /> After labeling, each sample will then be prepared for storage and shipment in accordance with <br /> the storage and preservation requirements listed for that method (see Section 4.1) and as further <br /> discussed in Section 6.3. <br /> Sample Chain-of-Custody Forms and Custody Seals <br /> AECOM will use standard sample chain-of-custody procedures to maintain and document sample <br /> integrity during collection, transportation, storage, and analysis. A sample will be considered to <br /> be in custody if one of the following statements applies: <br /> • It is in a person's physical possession or view <br /> • It is in a secure area with restricted access <br /> • It is placed in a container and secured with an official seal in such a way that the <br /> sample cannot be reached without breaking the seal <br /> The custody of a sample must be traceable from the time of sample collection to the reporting of <br /> results. Chain of custody procedures provide a mechanism for documenting information related <br /> to sample collection and handling. Completion of a chain of custody form must occur after sample <br /> collection and prior to sample shipment or release. Cross-checking the chain of custody form, <br /> sample labels, and field documentation is necessary to verify sample identification, date and time <br /> the sample was collected, the required containers and preservatives, and sample volume are in <br /> accordance with the project requirements. <br /> The chain-of-custody form will be used to document all samples collected and the analyses <br /> requested as part of this Project. Information that the field personnel will record on the chain-of- <br /> custody form includes the following: <br /> AECOM <br /> 17 <br />
