S 11 <br />1 N J OAQ U <br />I I N Environmental Health Department <br />COUNTY— AMENDED <br />1/9/2020 <br />Large Quantity Hazardous Waste Generator Inspection Report <br />Facility Name: Facility Address: Date: <br />PROFLEET TRUCK LUBE 1 15453 N THORNTON RD, LODI January 08, 2020 <br />SUMMARY OF VIOLATIONS <br />CLASS I,CLASS II,or MINOR-Notice to Comply) <br />Item# Remarks <br />115 CCR 66262.42(a)(b)(d)Failed to comply with uniform hazardous waste manifest exception requirements. <br />OBSERVATION: The facility did not have an exception report on site for Uniform Hazardous Waste Manifest <br />UHWM)number 018237080JJK(6/13/2019). <br />REGULATION: <br />a)A generator who does not receive a copy of the manifest with the handwritten signature of the owner or operator <br />of the designated facility within 35 days of the date the waste was accepted by the initial transporter shall contact the <br />transporter and/or the owner or operator of the designated facility to determine the status of the hazardous waste. <br />b)A generator shall submit an Exception Report to the Department if the generator has not received a copy of the <br />manifest with the handwritten signature of the owner or operator of the designated facility within 45 days of the date <br />the waste was accepted by the initial transporter.The Exception Report shall include: <br />1)a legible copy of the manifest for which the generator does not have confirmation of delivery; <br />2)a cover letter signed by the generator or the generator's authorized representative explaining the efforts taken to <br />locate the hazardous waste and the results of those efforts. <br />d)Generators shall submit the exception report to the department at: DTSC Report Repository, Generator <br />Information Services Section, P.O. Box 806, Sacramento, CA 95812-0806 <br />CORRECTIVE ACTION: Locate a copy of the destination facility signed manifest and send a copy to the San <br />Joaquin County Environmental Health Department(EHD). If the destination facility signed manifest cannot be <br />found then prepare an exception report for the missing manifest as required by Title 22 California Code of <br />Regulations section 66262.42. The exception report shall include a legible copy of the manifest for which the <br />generator does not have confirmation of delivery and a cover letter signed by the generator or the generator's <br />authorized representative explaining the efforts taken to locate the hazardous waste and the results of those efforts. <br />DTSC address is DTSC Report Repository, Generator Information Services Section, PO Box 806, Sacramento, CA <br />95812-0806.
