Disposal Facility (Site) Inspection Report (52) <br /> Forward Landfill, Inc. (39-AA-0015) <br /> 6/16/2026 <br /> Notes: <br /> 1. The disposal area was recently relocated to WMU-24 Phase 2. Daylighting observed at the <br /> previous disposal area. Per operator, they are currently working on covering the areas. <br /> 2. The Water Board cleared the site for the Non-Confirming waste on June 10, 2026 and the <br /> site can begin accepting waste from Kirkwood. <br /> 3. On June 5, 2026, the LEA was informed by the operator of a tear in the liner of WMU-24 <br /> Phase 2. Operator is in contact with the Water Board. The incident was documented in the <br /> Special Occurrence Log. <br /> 4. There is currently one radiation load isolated at the facility. <br /> 5. An excess amount of plastic sheets were observed on the ground of the designated Special <br /> Waste area. Operator was informed to dispose of the plastic sheets at the active face. <br /> 6. Operator shall contact CalRecycle for clarification on the removal of the dirt from the <br /> southern leachate pond. <br /> 7. Tarps, overs and soil are used for daily cover. <br /> 8. Track out and odors were light. <br /> 9. Records were reviewed. <br /> 10. The gas meter was calibrated on the morning of the inspection. Perimeter landfill gas <br /> monitoring probes GP-4, GP-5 and GP-6 were measured today. Results were 0.0%, below the <br /> regulatory limit of 5% methane. See the attached log with complete results and calibration. <br /> 11. Gas monitoring well GP-6 was not locked or secure.
