CP26 - UST FULL Inspection Report Date: June 30, 2026 Facility Address: 351 N BECKMAN RD, LODIVAN DE POL ENT INC/PACIFIC PRIDE Facility Name: Environmental Health Department SUMMARY OF VIOLATIONS (CLASS I, CLASS II, or MINOR - Notice to Comply) Item #Remarks 1 23 CCR 16 2691(b)-(f); HSC 6.7 25284 and 25287 2011010 - Facility obtained valid Permit to Operate from the UPA, complies with requirements and has paid fees OBSERVATION: Owner/Operator failed to obtain and/or retain a valid permit to operate from the UPA, and/or failed to comply with one or more permit requirements, and/or failed to pay permit fees. - The permit requirements indicate that a current permit, response plan, monitoring site plan and monitoring plans must be on site. None of the aforementioned documents were available for review. REGULATION GUIDANCE: Failure to obtain or retain a valid permit to operate from the UPA and comply with the applicable requirements of the permit, including but not limited to paying all applicable local government fees and state surcharge fees. CORRECTIVE ACTION: If Owner/Operator has not obtained a valid permit to operate, or if the permit to operate has expired, a complete application to obtain a current permit to operate must be submitted to the UPA for approval. If facility is not in compliance with one or more requirements of the permit to operate, owner/operator must submit documentation of compliance. All unpaid permit fees must be paid. All applicable violations must be corrected within 30 days. This is a Repeat (Class 2) Violation. 2 23 CCR 16 2613(a)(1)-(4) and (6), and (b), 2690; HSC 6.7 25286 2011009 - Complete and accurate application for a permit to operate a UST has been submitted and maintained OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and/or accurate application for a permit to operate a UST or failed submit application for renewal permit. - An application for a permit to operate a UST has not been submitted REGULATION GUIDANCE: "Failure to submit and maintain a complete and accurate application for a permit to operate a UST, or for renewal of the permit, that includes the following information: 1) Tank Owner 2) Facility Information 3) Tank Operator 4) Property Owner 5) UST Information 6) Tank Use and Contents 7) Name of supervisor of the division, section, or office that operates the UST, if the owner or operator is a public agency" CORRECTIVE ACTION: Submit and/or maintain a complete and/or accurate application for a permit to operate a UST, including for permit renewal. This is a (Minor) Violation. 3 23 CCR 16 2613(a)(3) and (b), 2650(a), (b), and (e) 2011011 - Facility has submitted and maintains a complete and accurate UST Monitoring Plan OBSERVATION: Owner/Operator failed to submit and/or maintain a complete and accurate UST Monitoring Plan. - A UST Monitoring Plan has not been submitted REGULATION GUIDANCE: Failure to submit or maintain a complete and accurate UST Monitoring Plan for approval by the UPA.
