Amendments <br />were made to the checklist and are listed below. <br /> A complete report was finished on July 23, 2025 and replaces the initial checklist. <br />Complete and submit a copy of the Return to Compliance Certification form to the EHD with a statement <br />documenting the corrective actions that have been or will be taken for each violation, and any supporting paperwork, <br />within 30 days of receiving the complete inspection report. <br />Amendments made to checklist: <br />- Added "Repeat" to Violation #10 <br />- Removed violation #13 <br />The report was amended on September 15, 2025 and replaces the initial report. <br />Amendments made to the July 22, 2025 report: <br />- Violations 3, 4, 11, 14 were upgraded from a MINOR violation to a Class 2 violation <br />1868 E.
