The report states that hand auger <br /> borings were drilled. <br /> Pursuant to the Code of San Joaquin County, Section 5-4000 et seq., a permit shall be secured for all test <br /> holes or soil borings. Work at the above mentioned site was completed without permit or guidance from j <br /> PHS/EHD. Therefore, enclosed is a permit application to be completed by you and returned to this <br /> department along with the permit and penalty fees totalling$178.00. Of primary concernto this department <br /> is the destruction of soil borings, especially at sites with known contamination. Since no inspector was <br /> present for the soil borings at this site, documentation must be submitted stating how the holes were <br /> destroyed. <br /> Prior to any future work being done at this site, 48 hours advance notice must be given and any necessary <br /> permits must be obtained from this department. <br /> Submit the application, fees, and description of the boring destruction to this department by October 7, <br /> 1991. <br /> If there are any questions, contact Linda Turkatte, EHS,at (209)468-3442. <br /> Jogi Khanna, M.D., M.P.H. <br /> Health Offic <br /> ne A.
