Contact the local fire department and the San Joaquin Valley Air Pollution Control District for <br />any applicable permits and requirements. <br />2. Any changes in information and scope of work must be submitted to EHD for approval prior to <br />commencing work. All new addenda supersede the previous ones. <br />3. Provide written notification to ustCa�slgov.org to arrange the following inspection appointments <br />(a 48-hour notice is required for inspection appointments). <br />a. Removal of tank and any applicable piping and components <br />b. Verification that tank is cleaned to Title 22 CCR 67383.3(e)(2) standards <br />c. Witness soil and/or water sampling <br />d. Verification of tank ID number marked on the tank end using fluorescent spray paint <br />4. If the removed backfill material and soil is found to be contaminated it must be disposed of at <br />an approved disposal location. Treatment of soil on site can only be performed under a <br />hazardous waste treatment permit. <br />5. The contractor shall provide the following: <br />a. Combustible/Flammable gas detector (to verify the lower explosive limit (LEL) <br />atmosphere and oxygen level of tank prior to lifting from excavation) <br />b. Adequate number(s) and appropriate type(s) of fire extinguisher(s) <br />c. Barriers to secure the area as necessary to minimize traffic and pedestrian interference <br />d. Fluorescent spray paint to affix tank identification numbers <br />6. A copy of the submitted "Health and Safety Plan" must be on the jobsite for review. <br />7.
