COMPLAINT INSPECTION REPORT <br /> 4:30 pm <br /> 4:00 pm <br />Time Out: <br />Time In: <br />Environmental Health Department <br />Name of Facility: CHOP STICK Date: 12/02/2021 <br />Address: 1304 N EL DORADO ST, STOCKTON 95202 <br />Owner/Operator: HO, MEI YI Telephone: (209) 466-5001 <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO0054423 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COMPLAINANT WENT TO FACILITY ON 10/30/2021 AT 3:30PM AND ORDERED WON TON SOUP AND SALT AND <br />PEPPER SHRIMP. COMPLAINANT DID NOT SEE ANY HAND WASHING. <br />WHEN COMPLAINANT WALKED IN COMPLAINANT OBSERVED THROUGH A WINDOW TO THE KITCHEN, THREE <br />PEOPLE SITTING IN A CIRCLE ON BUCKETS - ONE OF THEM WAS RUBBING THEIR FEET WITH THEIR SHOES <br />OFF. A DIFFERENT EMPLOYEE CAME OUT FRONT AND TOOK THE COMPLAINANTS ORDER & PAYMENT. THE <br />EMPLOYEE WHO HAD BEEN RUBBING HER FEET GOT UP, PUT HER CROCS (SHOES) BACK ON, AND BEGAN <br />PREPARING THE COMPLAINANT'S FOOD WITHOUT WASHING HER HANDS. COMPLAINANT LET THE CASHIER <br />KNOW. COMPLAINANT STATES THE CASHIER WENT INTO THE KITCHEN, RINSED OFF THE COMPLAINT'S FOOD, <br />AND PUT THEM INTO A POT-WITHOUT WASHING HER HANDS AFTER HANDLING MONEY. COMPLAINANT LEFT. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate closure of <br />the food facility. <br />OVERALL INSPECTION NOTES AND COMMENTS <br />No Temperature Data Collected <br />FOOD ITEM -- LOCATION -- TEMP º F -- COMMENTS <br />NOTES <br />Re-Inspection at facility to verify corrections for items observed out of compliance during he 11-10-2021 inspection. Hand <br />soap and paper towel dispensers were obtained for the restroom hand sink. Bathroom tissue was obtained for the restroom. <br />Operator stated the bathroom tissue dispenser was installed behind the restroom stall door during the inspection. But installed <br />a new one on the side of the toilet. Verified all corrections were done. <br />The person in charge is responsible for ensuring that the above mentioned facility is in compliance with all applicable sections of the California Health and <br />Safety Code.
