COMPLAINT INSPECTION REPORT <br />Environmental Health Department <br />Name of Facility: Date: 05/07/2025 <br />Address: 4200 E FREMONT , STOCKTON <br />Owner/Operator: Telephone: () - <br />Program Element: 1600 - FOOD PROGRAM Complaint #: CO2500276 <br />Responsible Party Contact: <br />DESCRIPTION OF COMPLAINT ALLEGATIONS <br />The San Joaquin County Environmental Health Department received the following complaint: <br />COCKROACHES INSIDE AND OUTSIDE THE TRUCK. <br />VIOLATIONS AND CORRECTIVE ACTIONS <br />Items listed on this report as violations do not meet the requirements set forth in the California Health and Safety Code commencing with section 7; <br />113700. All violations must be corrected within specified timeframe. Violations that are classified as "MAJOR" pose an immediate threat to public health <br />and have the potential to cause foodborne illness. All major violations must be corrected immediately. Non-compliance may warrant immediate <br />closure of the food facility. <br /> #7 Hot and Cold Holding Temperatures <br />OBSERVATIONS: The following unit for cold storage of Potentially Hazardous Foods (PHFs) was observed over the required <br />temperature of 41 F: <br />- Prep cooler was observed at 65 F. <br />PHFs are required to be stored at 41 F or below. Adjust/repair unit to maintain all PHFs at 41 F or below. Correct today. <br />CALCODE DESCRIPTION: Potentially hazardous foods shall be held at or below 41/ 45°F or at or above 135°F. (113996, 113998, <br />114037, 114343(a)) <br />Major <br /> #23 Rodents, Insects or Animals Inside Facility <br />OBSERVATIONS: Observed several live cockroaches inside the cooler. Observed one cockroach that came out from the sliced <br />tomatoes. Deep clean and sanitize premises. Discard all contaminated food items. Remove all cockroaches using approved <br />methods safe for food facilities. Correct today. <br />This mobile food facility will close and correct violations. A reinspection will be done once violations have been corrected . <br />CALCODE DESCRIPTION: Each food facility shall be kept free of vermin: rodents (rats, mice), cockroaches, flies.( 114259.1, 114259.4, <br />114259.5) <br />Major <br /> #6 Handwashing Facilities Supplied and Accessible <br />OBSERVATIONS: The hand soap and paper towel dispensers were empty. Refill and maintain stocked during all hours of <br />operation. Correct today. <br />CALCODE DESCRIPTION: Handwashing soap and towels or drying device shall be provided in dispensers dispensers shall be <br />maintained in good repair.
