The <br />feedback below only reference violations that remain open, all violations not listed below have been <br />closed. <br />HMBP: <br />• Violation #12— <br />o ER Plan submitted to CERS: <br />■ 1 still don't see the locations and capabilities (single use, 1 month supply, etc.) of <br />the emergency response equipment (fire extinguishers, first aid kits, etc.) <br />■ Section II has our (EHD/local CUPA) phone number and address listed under the <br />Environmental Protection Agency. There is also a contact listed as "HAZMAT" but <br />it's not clear what that means. Is that internal? Another agency? Lastly, I don't see <br />the CaIOES spill reporting phone number, 800-852-7550. Please make the <br />necessary corrections and resubmit. <br />o Spill Response Plan submitted to CERS: <br />■ 1 also don't seethe CaIOES phone number or EHD/local CUPA phone numbers <br />listed here in the emergency contact list. <br />LQG HW: <br />• Violation #2 -1 see all the training records for fire and emergency preparedness, but I don't see <br />any topics specific to hazardous waste handling (labeling, storage, recordkeeping, disposal <br />records, accumulation time limits, etc.). Please provide those training records. <br />• Violation #11 — <br />o Observation 1 — <br />■ Received analytical report for filter cake and shaker screen. <br />■ RTC submitted 10/3/25 states the other waste streams "will be tested prior to <br />disposal which is done annually in February". Please provide documentation of the <br />determination for the remainingwaste streams, including: <br />• Washroom trench <br />• Wastewater pit <br />• Wastewater pit composite <br />• Sand/grit trap composite <br />• Pit & trench sludge <br />o Observation 2 — <br />
