Ms. Amy Terrell <br /> 20 August 2007 <br /> Page 4 <br /> Utility Clearance <br /> Prior to any subsurface drilling or injection activities, all proposed locations will be marked and <br /> checked for any potential subsurface utilities or obstructions. Injection points and new <br /> monitoring well locations will be marked with white paint and with physical markers (e.g. <br /> feathers, stakes, etc.) if necessary. Underground Service Alert (USA) will be contacted a <br /> minimum of 48 hours prior to field work activities. Additionally, the test plot in the northern <br /> portion of the Site will be surveyed by a private utility locator. If potential subsurface <br /> obstructions are encountered during the course of the utility survey, the proposed injections will <br /> be moved to clear identified obstructions. <br /> Health and Safety <br /> All field operations will be performed using the procedures and equipment outlined in the site- <br /> specific Health and Safety Plan (HASP) included as Attachment 1. A safety tailgate meeting will <br /> be held prior to the start of field activities each day. <br /> Monitoring Well Destruction <br /> As reported previously to the RWQCB, the surface casing of MW-6 is bent and the well is no <br /> longer accessible for sampling. MW-6 will be destroyed by over-drilling the casing and pressure <br /> grouting to ground surface. Permitting, destruction, grouting, and inspection will be conducted <br /> in accordance with SJF,HD requirements. <br /> Monitoring Well Installation <br /> Two new monitoring wells will be installed at the Site prior to the HRC-X® injections: MW-6R <br /> and MW-7. MW-6R is being installed to replace MW-6 which had its surface casing bent and is <br /> no longer accessible for sampling. The location of MW-6R will be southwest of MW-6, <br /> coinciding with the center of the test plot located at the northern portion of the Site (Figure 2). <br /> The purpose of this new well is to measure changes in groundwater quality in the center of the <br /> HRC-X® injection grid. MW-7 is proposed to be installed southeast of the test plot to monitor <br /> groundwater quality downgradient of the test plot area and MW-2 will monitor downgradient <br /> quality northeast of the test plot.
