During a site visit, it was <br /> determined that the presence of utility lines render completion of the soil boring infeasible. During a conversation held earlier this <br /> week between myself and Mr. Michael Infurna of the County of San Joaquin Department of Environmental Health, Mr. Infuma <br /> requested that the well vault for monitoring well MW-9(located approximately 5 feet west of ATC's proposed soil boring location) <br /> be raised up to the road surface. <br /> Accordingly, ATC is requesting modification of the attached encroachment permit(Permit No. PS-1101283) to perform the well <br /> vault repair activities. The scope of work would involve removing a 2-foot square section of asphalt which surrounds the sunken <br /> well vault, and replacing the well vault with a new one set in concrete at an elevation flush with the surrounding pavement. At the <br /> conclusion of the environmental investigation when the monitoring well is destroyed, the concrete pad would be removed and an <br /> asphalt hot patch would be emplaced. The existing traffic control plan is sufficient to perform the work. <br /> If you have any questions, please don't hesitate to call me at (805) 496-1217.
