PS-1102769 1-26-12 SPECIAL CONDITIONS FOR CVIN - SEGMENT 16 ENCROACHMENT PERMIT 1. The conduit alignment shall be placed no closer than six (6) feet from the edge of pavement. 2. Minimum compaction in unpaved areas shall be 90%. 3. Compaction tests will be required for each day of trenching and shall be taken at locations as directed by the County Public Works inspector/field engineer. 4. The fiber optic cable is proposed to cross state and/or locally regulated channels at several locations. These crossings will require permits from the State (Central Valley Flood Protection Board - CVFPB) and/or San Joaquin County Flood Management Division - SJCFMD (Watercourse Encroachment Permit and/or Endorsement Letter). Required permits for each of the proposed crossings: • Duck Creek— (State Permit from CVFPB and an Endorsement Letter from SJCFMD) • South Potter Creek, Channel A— (Watercourse Encroachment Permit from SJCFMD) • Potter Creek, Channel A— (Watercourse Encroachment Permit from SJCFMD) • Mormon Slough — (State Permit from CVFPB and an Endorsement Letter from SJCFMD) • Tulsa Drain — (Watercourse Encroachment Permit from SJCFMD) CVIN shall apply directly to the Central Valley Flood Protection Board for the state permits, but first submit a copy of the application(s) directly to the San Joaquin County Flood Management Division for the associated Endorsement Letter(s) (one for each crossing). The CVFPB will not issue their permit(s) without first receiving SJCFMD Endorsement Letter(s). In addition, the applicant must apply directly to the SJCFMD for the required Watercourse Encroachment Permit(s). Fees for each item apply; per current San Joaquin County Public Works Fee Schedule. "No work shall occur within 30 feet of the top of bank or 30 feet from the landside toe of the levee (where a levee exists) until such time all required State and County permits are obtained as confirmed by the Flood Management Division of the San Joaquin County Dept. of Public Works (Phone: 209-468-3605)." 5. Maintain traffic controls for all roads. Traffic control delays shall not exceed 15 minutes. Two-way traffic shall be maintained during non-working hours with excavated areas backfilled or plated. During working hours, two-way traffic control shall be maintained with one lane open and appropriate flaggers. 6. Access to all properties shall be maintained at all times except when work is occurring at the access point. Minimal delays will be allowed to provide access within the work zone area. Driveway access shall be fully restored at the end of each workday. Driveways disturbed by the contractor shall be replaced with in-kind or better materials.
