GENERAL NOTES: <br />1. Where shown on the Plans, 0.17' HMA overlay must be placed in one (1) layer <br />on the existing surface. <br />2. Where shown on the Plans, 0.30' HMA overlay must be placed in two (2) layers <br />of approximately equal thickness on the prepared surface. <br />3. Pavement core data is unavailable, however, anticipate varying layers of base and <br />roadway surfacing materials within the sectional planing depth of the existing <br />roadway. <br />4. Placement of HMA Overlay must not begin until the HMA used in Pavement <br />Repair has been in place for a minimum of 18 hours. <br />5. Placement of HMA Driveways must not begin until the HMA used for Mainline <br />Paving has been completed. <br />6. Existing highway and utility facilities must remain in place unless otherwise noted. <br />7. Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />8. Existing drainage facilities must remain in place unless otherwise noted. The <br />Contractor must conduct earthwork operations so as not to modify or obstruct <br />the existing drainage. <br />9. Hand grinding of roadway surfacing will be required at locations where existing <br />highway and utility facilities are within the Cold Plane Pavement or Remove <br />Subbase, Base, and Surfacing limits. Payment for hand grinding will be considered <br />as included in the contract unit price for Cold Plane Pavement or Remove <br />Subbase, Base, and Surfacing. <br />10. All Earthwork, except for final shoulder grading, must be completed prior to <br />placing the final layer of NMA pavement. No Shoulder Backing material will be <br />deposited or drifted on the final layer of HMA surfacing. Where the shoulder <br />placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in <br />two (2) or more fully compacted layers. <br />11. Shoulder Backing material at all intersection radii must receive Penetration <br />Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work <br />must be confined to intersection radii plus fifty feet (50') in each direction or to <br />the intersection conform at 4' width. <br />12. The Contractor will be responsible for adequately scheduling inspection and testing <br />of all facilities constructed under this contract with a minimum 24 hour prior <br />notice to the County. All testing must conform to the Standard Specifications. The Contractor must obtain all permits and licenses required for the construction <br />and completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the <br />project. <br />14. The Contractor must enter into an Agreement with the Burlington Northern Santa <br />Fe Railway Company for any work within the Railroad's right—of—way. See Section <br />5-1.20C Railroad Relations of the Specifications for further details. <br />15. The Contractor must exercise due caution and carefully preserve survey bench <br />marks, control points, reference points and all survey stakes, and must bear all <br />expenses for replacement and/or errors caused by their unnecessary loss or <br />disturbance. <br />16. Prior to commencing any work, it will be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor must notify members of the Underground Service Alert (USA) 48 hours <br />in advance of performing excavation work by calling the toll free number <br />(1-800-642-2444) or 811. <br />17. The Contractor must field verify all existing highway and utility facilities' locations <br />and depths. It will be the Contractor's sole responsibility to locate and protect all <br />existing highway and utility facilities, so that no damage results to them during <br />the performance of this contract. The Contactor will be required to cooperate <br />with other contractors and utility companies installing new structures, utilities and <br />services to the project. Payment for this work will be considered to be included <br />in the various items of work involved. <br />18. Roadways, or segments thereof, that are to have the surface removed may be <br />temporarily closed during construction activities, but remain open for local traffic. <br />Schedule of closure(s) must be approved by the Engineer. The Contractor must <br />provide two weeks advance notice to all affected residents, services, schools, etc. <br />of the scheduled closure. Content of the public notification must be approved by <br />the Engineer. <br />19. The Contractor must place temporary ramps to affected residents' driveways at <br />the end of each working day if a drop off greater than 0.15' exists, Ramp <br />materials must be consistent with that of the roadbed (or roadway) surface that <br />is to be placed upon.
