PS-1300912 April 4,2013 <br /> SPECIAL CONDITIONS FOR <br /> MACDONALD ISLAND ROAD RE-ALIGNMENT <br /> ENCROACHMENT PERMIT <br /> 1. Maintain traffic controls for all roads. Traffic control delays shall not exceed 15 minutes. <br /> Two-way traffic shall be maintained during non-working hours with excavated areas backfilled or <br /> plated. During working hours, two-way traffic control shall be maintained with one lane open and <br /> appropriate flaggers. No road closures are authorized by this encroachment permit. <br /> 2. The traffic shift shall occur under traffic control. <br /> 3. Access to all properties shall be maintained at all times except when work is occurring at the <br /> access point. Minimal delays will be allowed to provide access within the work zone area. <br /> Driveway access shall be fully restored at the end of each workday. Driveways disturbed by the <br /> contractor shall be replaced with in-kind or better materials. <br /> 4. Applicant shall provide the County verification from the property owner(s) granting an easement <br /> for new roadway alignment. <br /> 5. The right-of-way process to the County for road easement shall be completed within six months <br /> of final roadway surfacing. <br /> 6. Final acceptance of project will not occur until the right-of-way process is complete and the grant <br /> of easement to the County has been recorded. <br /> 7. Reclamation District shall be responsible for road maintenance until the right-of-way process is <br /> completed and recorded. <br /> 8. Subgrade and base rock grade shall be constructed by Reclamation District and an oiled surface <br /> (dust control) shall be placed before traffic is shifted to new alignment. <br /> 9. If settlement occurs after two months, the Reclamation District shall regrade roadway and <br /> shoulder prior to placement of final surfacing. Final determination shall be made by County <br /> Engineer. <br /> 10. Flexible delineators shall be placed at 100-foot intervals along new edge of travel way on both <br /> sides of road. <br /> 11. The permittee is responsible for any Survey Monuments disturbed by this project and shall file a <br /> corner record to re-establish them using a Licensed Land Surveyor. <br /> 12. Roadside ditches shall be graded to drain towards proposed culverts. <br />
