1. <br />2. <br />3. <br />4. <br />5. <br />6. <br />7. <br />,1 <br />E <br />GENERAL NOTES: <br />0.25' HMA Overlay must be placed in two (2) layers of approximately equal thickness. <br />Core data is unavailable; however, anticipate varying layers of base and roadway <br />surfacing materials within the sectional planing depth of the existing roadway. <br />Placement of HMA Overlay must not begin until the HMA used in Pavement Repair has <br />been in place for a minimum of 18 hours. <br />Placement of HMA driveways must not begin until the HMA used for Mainline Paving has <br />been completed. <br />Existing highway and utility facilities must remain in place unless otherwise noted. <br />Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain <br />in place unless otherwise noted. <br />Existing drainage facilities must remain in place unless otherwise noted. The Contractor <br />must conduct earthwork operations so as not to modify or obstruct the existing <br />drainage. <br />All Earthwork, except for final shoulder grading, must be completed prior to placing the <br />final layer of HMA pavement. No Shoulder Backing material will be deposited or drifted <br />on the final layer of HMA surfacing. Where the shoulder placement exceeds 0.50' in <br />thickness, the Shoulder Backing must be placed in two (2) or more fully compacted <br />layers. <br />Shoulder Backing material at all intersection radii must receive Penetration Treatment at <br />0.25 gallon/square yard. The Contractor will be responsible for adequately scheduling inspection and testing of all <br />facilities constructed under this contract with a minimum 24 hour prior notice to the <br />County. All testing must conform to the Standard Specifications. All re—testing will be <br />paid by the Contractor. <br />11. Prior to commencing any work, it will be the Contractor's responsibility to have each <br />utility company locate in the field their main service lines and laterals. The Contractor <br />must notify members of the Underground Service Alert (U.S.A.) 48 hours in advance of <br />performing excavation work by calling the toll free number (1-800-642-2444) or 811. <br />12. The Contractor must exercise due caution and carefully preserve survey bench marks, <br />control points, reference points and all survey stakes, and must bear all expenses for <br />replacement and/or errors caused by their unnecessary loss or disturbance. <br />13. The road will be closed to through public traffic; however, access to local residents will <br />be maintained at all times. Detour signs will be placed by County forces and maintained <br />by the Contractor until the project is completed. <br />14. Roadway, or segments thereof, that are less than 20 feet in paved width may be <br />temporarily closed to all traffic during construction activities. Schedule of closure(s) must <br />be approved by the Engineer. Contractor must provide a two weeks advance notice to all <br />affected residents, services, schools, etc. of the scheduled closure. Content of public <br />notification must be approved by the Engineer. <br />15. The Contractor must place temporary ramps to affected residents' driveways at the end <br />of each working day if a drop off greater than 0.15' exists.
