See reverse side and attached sheet, if any. <br />Trench excavations for service connections will not be pemύtted within ten feet ( 1 Ο') of pavement centerline unless otherwise approved by the <br />Director. Surface of trench patches shall match in kind and be smooth and even with that of abutting surface. Special attention shall be given <br />to depth ofutilities through roadside area in anticipation offuture drainage facilities, road profile and/or frontage development. All <br />undeigωund utility facilities aι·e to be established and accurately dirnensioned on slcetches fiΌm surveyed centerline of road right ofway, ΟΙ <br />from Iight ofway (border) lines. <br />Permittee shall call the Department of Public Works, Field Engineering Division (Permit Inspections) at (209)953-7421 at least <br />forty-eight hours prior to beginning any work within the County right ofway. All worlc performed under this pemύt shall conform to <br />the rules and regulations pertaining to safety established by the Califoιηia Division ofindustrial Safety and Cal-OSHA. <br />The jobsite shall be lcept in a safe condition at all times by the daily removal of any excess dirt 01· debris which might be a hazard to either <br />pedestrian or automobile tra:ffic. All necessary tra:ffic convenience and warning devices and persoIJ11e! shall be provided, placed and <br />maintained by and at the sole expense ofthe Pemύttee in accordance with the latest edition ofthe CALTRANS Manual ofTra:ffic Control. <br />AfteI completion ofthe worlc pemύtted heiein, all debris, lumber, baιτicades, or any excess mateiial shall be Iemoved and the jobsite left in a <br />neat woilcmanlilce maIJ11er'.
