GENERAL NOTES: <br />1. 0.25' Hot Mix Asphalt Overlay must be placed in two (2) layers of approximately equal <br />thickness, with Geosynthetic Pavement Interlayer (Paving Fabric) placed between the <br />layers. <br />2. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix Asphalt used <br />in Pavement Repair has been in place for a minimum of 18 hours. <br />3. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix Asphalt used <br />for Mainline Paving has been completed. <br />4. Existing highway and utility facilities must remain in place unless otherwise noted. <br />5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain <br />in place unless otherwise noted. <br />S. Existing drainage facilities must remain in place unless otherwise noted. Contractor must <br />conduct Earthwork operations so as not to modify or obstruct the existing drainage. <br />7. All Earthwork, except for final shoulder grading, must be completed prior to placing the <br />final layer of Hot Mix Asphalt Pavement. No shoulder backing material will be deposited or <br />drifted on the final layer of Hot Mix Asphalt surfacing. Where shoulder placement exceeds <br />0.50' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted <br />layers. <br />8. Shoulder Backing material at all intersection radii must receive Penetration <br />Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work <br />must be confined to intersection radii plus fifty feet (50') in each direction or <br />to the intersection conform. <br />9. The Contractor will be responsible for adequately scheduling and testing of all facilities <br />constructed under this contract with a minimum 24 hour notice to the County. All testing <br />must conform to the Standard Specifications. All re—testing will be paid by the Contractor. <br />10. The Contractor must obtain all permits and licenses required for the construction and <br />completion of the project, and must perform all work in accordance with the requirements <br />and conditions of all permits and approval applicable to the project. <br />11. Hand grinding of Asphalt Concrete pavement will be required at locations where the <br />existing highway facilities are within the Planing Asphalt Concrete Pavement and Pavement <br />Repair limits. Payment for hand grinding will be considered as included in the contract unit <br />piice paid for Planing Asphalt Concrete Pavement, <br />12. The Contractor must exercise due caution to carefully preserve survey bench marks, <br />control points, reference points and all survey stakes, and must bear all expenses for <br />replacement and/or errors caused by their unnecessary loss or disturbance. <br />D 13. Prior to commencing any work, it will be the Contractors responsibility to have each <br />m utility company locate in the field their main service lines and laterals. The Contractor must field verify all existing highway and utility facilitieslocations <br />and depths. It will be the Contractors sole responsibility to locate and protect all existing <br />highway and utility facilities, so that no damage results to them during the performance of <br />this contract and utility companies installing new structures, utilities, and services to the <br />project. Payment for this work be considered as included in the the various items of work. <br />15. Existing Maintenance <br />Holes are surrounded by paved pads of various <br />sizes. The Contractor <br />must place new Hot Mix <br />Asphalt <br />matching new grade of pavement, prior <br />to Adjust Frame and <br />Cover (SSJID Maintenance <br />Hole). <br />See Sheet No. 6 for details. Placement <br />of Hot Mix Asphalt <br />will be considered as included <br />in <br />the various items of work, <br />17+80 <br />16. Cross drain and driveway pipes must be cleaned prior to placing final layer of <br />Hot Mix Asphalt Pavement. Cleaning of existing pipes will be considered as included in the <br />Cleaning Existing Drainage Facilities item of work. Pipe located at Station 82+57 (Left) is to be extended six feet (6') per PIPE DETAIL <br />on Sheet No. 5. The 24" Corrugated Steel Pipe pay item includes payment for the partial <br />removal of the existing pipe and installation of the new pipe, including all necessary <br />materials.
