GENERAL NOTES: 1. 0.25' Hot Mix Asphalt Overlay must be placed in two (2) layers of approximately equal thickness, with Geosynthetic Pavement Interlayer (Paving Fabric) placed between the layers. 2. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix Asphalt used in Pavement Repair has been in place for a minimum of 18 hours. 3. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix Asphalt used for Mainline Paving has been completed. 4. Existing highway and utility facilities must remain in place unless otherwise noted. 5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain in place unless otherwise noted. S. Existing drainage facilities must remain in place unless otherwise noted. Contractor must conduct Earthwork operations so as not to modify or obstruct the existing drainage. 7. All Earthwork, except for final shoulder grading, must be completed prior to placing the final layer of Hot Mix Asphalt Pavement. No shoulder backing material will be deposited or drifted on the final layer of Hot Mix Asphalt surfacing. Where shoulder placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted layers. 8. Shoulder Backing material at all intersection radii must receive Penetration Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be confined to intersection radii plus fifty feet (50') in each direction or to the intersection conform. 9. The Contractor will be responsible for adequately scheduling and testing of all facilities constructed under this contract with a minimum 24 hour notice to the County. All testing must conform to the Standard Specifications. All re—testing will be paid by the Contractor. 10. The Contractor must obtain all permits and licenses required for the construction and completion of the project, and must perform all work in accordance with the requirements and conditions of all permits and approval applicable to the project. 11. Hand grinding of Asphalt Concrete pavement will be required at locations where the existing highway facilities are within the Planing Asphalt Concrete Pavement and Pavement Repair limits. Payment for hand grinding will be considered as included in the contract unit piice paid for Planing Asphalt Concrete Pavement, 12. The Contractor must exercise due caution to carefully preserve survey bench marks, control points, reference points and all survey stakes, and must bear all expenses for replacement and/or errors caused by their unnecessary loss or disturbance. D 13. Prior to commencing any work, it will be the Contractors responsibility to have each m utility company locate in the field their main service lines and laterals. The Contractor must field verify all existing highway and utility facilitieslocations and depths. It will be the Contractors sole responsibility to locate and protect all existing highway and utility facilities, so that no damage results to them during the performance of this contract and utility companies installing new structures, utilities, and services to the project. Payment for this work be considered as included in the the various items of work. 15. Existing Maintenance Holes are surrounded by paved pads of various sizes. The Contractor must place new Hot Mix Asphalt matching new grade of pavement, prior to Adjust Frame and Cover (SSJID Maintenance Hole). See Sheet No. 6 for details. Placement of Hot Mix Asphalt will be considered as included in the various items of work, 17+80 16. Cross drain and driveway pipes must be cleaned prior to placing final layer of Hot Mix Asphalt Pavement. Cleaning of existing pipes will be considered as included in the Cleaning Existing Drainage Facilities item of work. Location of pipes are approximate and must be field verified by the Contractor. Pipe Location: Sta 17. Pipe located at Station 82+57 (Left) is to be extended six feet (6') per PIPE DETAIL on Sheet No. 5. The 24" Corrugated Steel Pipe pay item includes payment for the partial removal of the existing pipe and installation of the new pipe, including all necessary materials. Associated earthwork and placement of shoulder backing material will be considered as included in the Earthwork (Misc Areas), end Shoulder Backing pay items, respectively.
