0 <br />GENERAL NOTES <br />1. 0.17Hot Mix Asphalt Overlay must be placed in one (1) layer with Geosynthetic <br />Pavement Interlayer (Paving Fabric) placed on existing or prepared surface. <br />2. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix Asphalt used <br />in Pavement Repair has been in place for a minimum of 18 hours. <br />3. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix Asphalt <br />used for Mainline Paving has been completed. <br />4. Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />5. All Earthwork, except for final shoulder grading, must be completed prior to placing <br />the final layer of Hot Mix Asphalt Pavement. No Shoulder Backing material will be <br />deposited or drifted on the final layer of Hot Mix Asphalt surfacing. Where the <br />shoulder placement exceeds 0.50' in thickness, the Shoulder Backing must be placed <br />in two (2) or more fully compacted layers. <br />6. Shoulder Backing material at all intersection radii must receive Penetration Treatment <br />at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be <br />confined to intersection radii plus fifty feet (50') in each direction or to the <br />intersection conform. <br />EP <br />7. The Contractor will be responsible <br />for adequately scheduling inspection <br />and testing of <br />all facilities <br />constructed under this <br />contract with a minimum 24 hour <br />notice to the <br />County. All <br />testing must conform <br />to the Standard Specifications. All <br />re—testing will <br />be paid by <br />the Contractor. <br />8. The Contractor must obtain all permits and licenses required for the construction <br />and completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />Areas in County right—of—way used by the Contractor for staging and/or storage of <br />materials must be approved by the Engineer. The areas utilized by the Contractor <br />for staging and/or storage of materials must be restored to their original condition. <br />Any damages or unclean condition to staging or storage areas must be repaired at <br />the Contractor's own expense. It will be the Contractor's responsibility to obtain <br />permission from the property owner to stage equipment or store materials on <br />private property.
