1. Prior to commencing any work, it will be the Contractors responsibility to have each <br />utility company locate in the field their main service lines and laterals. The Contractor <br />must notify members of the Underground Service Alert (U.S.A.) 48 Hours in advance of <br />performing excavation work by calling the toll free number (1-800-227-2600) <br />2. The Contractor must field verify all existing highway and utility facilitieslocations and <br />depths. It will be the Contractors sole responsibility to locate and protect all existing <br />highway and utility facilities, so that no damage results to them during the performance <br />of this contract. <br />3. The Contractor must obtain all permits and licenses required for the construction and <br />completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />4. Placement of temporary "No Parking" Signs must be placed no less than 24 hours prior <br />to commencing planing. <br />5. The Contractor must exercise due caution and carefully preserve survey bench marks, <br />control points, reference points and all survey stakes, and must bear all expenses for <br />replacement and/or errors caused by their unnecessary loss or disturbance. <br />6. The Contractor must place temporary ramps to affected residents' driveways at the end <br />of each working day if a drop off greater than 0.15' exists. Ramp materials must be <br />consistent with that of the roadbed (or roadway) surface that it is to be placed upon. <br />Payment for this work will be considered as included in the various items of work. <br />7. Hand Grinding of Asphalt Concrete Pavement will be required at locations where existing <br />highway and utility facilities are within the Planing Asphalt Concrete Pavement and <br />Pavement Repair limits. Payment for hand grinding will be considered as included in the <br />contract unit price paid for Planing Asphalt Concrete Pavement. <br />8. 0.33' Hot Mix Asphalt Overlay must be placed in two (2) layers of approximately equal <br />thickness with Geosynthetic Pavement Interlayer (Paving Fabric) placed on the existing or <br />prepared surface. <br />9. Placement <br />of Hot <br />Mix <br />Asphalt <br />Overlay must not begin <br />until <br />the Hot <br />Mix Asphalt used in <br />Pavement <br />Repair <br />has <br />been in <br />place fora minimum of <br />18 <br />hours. <br />10.
