GENERAL NOTES <br />1. Prior to commencing any work, it shall be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor shall notify members of the Underground Service Alert (USA) 48 hours in <br />advance of performing excavation work and/or sign relocation work by calling the <br />toll—free number 1-800-227-2600 or 811. <br />2, The Contractor shall field verify all existing highway and utility facilities' locations <br />and depths. It shaPl be the Contractors sole responsibility to locate and protect all <br />existing highway and utihity facilities, so that no damage results to them during the <br />performance of this contract. The Contractor shall be required to cooperate with <br />other contractors and utility companies installing new structures, utilities, and <br />services to the project. Payment for this work shall be considered to be included in <br />the various items of work. <br />3. The Contractor shall exercise due caution and shall carefully preserve survey bench <br />marks, control points, reference points and all survey stakes, and shall bear all <br />expenses for replacement and/or errors caused by their unnecessary loss or <br />disturbance. <br />4. Existing trees, shrubs, fences, mailboxes, utility poles and other frontage <br />improvements shall remain in .place unless otherwise noted. <br />5• The Contractor shall place temporary ramps to affected residents' driveways at <br />the end of each working day if a drop off greater than 0.15' exists. Ramp <br />materials shalF be consistent with that of the roadbed (or roadway) surface that <br />it is to be placed upon. Payment for this work shall be considered as included <br />in the various items of work. <br />6. The Contractor shal4 be responsible for adequately scheduling inspection and testing <br />of all facilities constructed under this contract. All testing shall conform to the <br />Standard Specifications. All re—testing shall be paid by the Contractor. <br />7. Shoulder Backing material at all intersection radii shall receive Penetration <br />Treatment at 0.25 gallons/square yard. Limits of the Penetration Treatment work <br />shall be confined to intersection radii plus twenty—five feet (25') in each direction <br />or to the intersection conform. <br />8. All earthwork, except for final shoulder grading, shall be completed prior to placing <br />the final layer of Asphalt Concrete Pavement. No Shoulder Backing material shall be <br />deposited or drifted on the final layer of Asphalt Concrete surfacing. Where the <br />shoulder placement exceeds 0.30' in thickness, the Shoulder Backing shall be placed <br />in two (2) or more fully compacted layers. <br />9. 0.33' Asphalt Concrete shall be placed in two (2) layers of approximately equal <br />thickness. <br />ECKED BY <br />DATE <br />10. The Contractor shad notify Jason Ender of City of Stockton (Public Works <br />Inspection) at (209) 937-8381 seventy—two (72) hours in advance of adjusting the <br />City of Stockton's facilities to finished grade.
