GENERAL NOTES: <br />1 . 0.17' HMA Overlay must be placed in one (1) layer on the existing or prepared <br />surface. <br />2. 0.25' HMA Overlay must be placed in two (2) layers of approximately equal thickness <br />on the existing or prepared surface, <br />3. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair has <br />been in place for a minimum of 18 hours, <br />4. Placement. of HMA Driveways must not begin until the HMA used for Mainline Paving <br />has been completed. <br />5. Existing highway and utility facilities must remain in place unless otherwise noted. <br />6, Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />7. Existing drainage facilities must remain in place unless otherwise noted. The <br />Contractor must conduct earthwork operations so as not to modify or obstruct the <br />existing drainage. <br />8, Hand grinding of Roadway Surfacing pavement will be required at locations where <br />existing highway and utility facilities are within the Cold Plane Pavement and Pavement <br />Repair limits: Payment for hand grinding will be considered as included in the <br />contract unit price paid for Cold Plane Pavement, <br />0, All Earthwork, except for final shoulder grading, must be completed prior to placing <br />the final layer of HMA pavement. No Shoulder Backing material will be deposited or <br />drifted on the final layer of HMA surfacing. Shoulder Backing material at all intersection radii must receive Penetration Treatment <br />at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be <br />confined to intersection radii plus fifty feet (50') in each direction or to the <br />intersection conform. <br />11. The Contractor will be responsible for adequately scheduling inspection and testing of <br />all facilities constructed under this contract with a minimum 24 hour prior notice to <br />the County. All testing must conform to the Standard Specifications. All re—testing will <br />be paid by the Contractor. <br />12. The Contractor must obtain an Encroachment Permit from the State Department of <br />Transportation for placement for performing traffic control or placing traffic control <br />devices in their jurisdiction. Payment for this will be considered to be included in the <br />various items of work involved. <br />13.
