GENERAL NOTES: <br />1. 0.17' HMA Overlay on Bradford Street must be placed in one layer on the prepared <br />surface. <br />2. 0.20' HMA Overlay on River Drive must be placed in one layer on the prepared <br />surface. <br />3. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair <br />has been in place for a minimum of 18 hours. <br />4. Existing highway and utility facilities must remain in place unless otherwise noted. <br />5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />6. Existing drainage facilities must remain in place unless otherwise noted. The <br />Contractor must conduct earthwork operations so as not to modify or obstruct the <br />existing drainage. <br />7. All Earthwork, except for final shoulder grading, must be completed prior to placing <br />the final layer of HMA Pavement. No Shoulder Backing material will be deposited or <br />drifted on the final layer of HMA surfacing. Where the shoulder placement exceeds <br />0.50' in thickness, the Shoulder Backing must be placed in two or more fully <br />compacted layers. <br />8. The Contractor will be responsible for adequately scheduling inspection and testing <br />of all facilities constructed under this contract with a minimum 24 hour notice to <br />the County. All testing must conform to the Standard Specifications. All re—testing <br />will be paid by the Contractor. <br />9. The Contractor must obtain an Encroachment Permit from City of Stockton and <br />other agencies necessary for traffic control or any work outside of the County's <br />jurisdiction. Payment for this will be considered to be included in the various items <br />of work. <br />10. The Contractor must notify the City of Stockton at (209) 937-8706 72 hours in <br />advance of adjusting the City of Stockton's facilities to finished grade. Refer to <br />the City of Stockton's Standards for adjusting facilities to grade. <br />11. The Contractor must obtain all permits and licenses required for the construction <br />and completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />12. All new Curb, Gutter and Sidewalk must conform to the existing flowline elevations <br />with a straight grade between conforms unless otherwise noted. <br />14. Install Expansion Joints at beginning and end of new Curb, Gutter and Sidewalk <br />and between new sidewalks and existing driveways. <br />15. Prior to commencing any work, it will be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor must notify members of the Underground Service Alert (USA) 48 hours <br />in advance of performing excavation work by calling the toll free number <br />(1-800-642-2444) or 811. <br />16. The Contractor must field verify all existing highway and utility facilities' locations <br />and depths. It will be the Contractor's sole responsibility to locate and protect all <br />existing highway and utility facilities, so that no damage results to them during the <br />performance of this contract. The Contactor will be required to cooperate with <br />other contractors and utility companies installing new structures, utilities and <br />services to the project.
