GENERAL NOTES: 1. Prior to commencing any work, it will be the Contractor's responsibility to have each utility company locate in the field their main service lines and laterals. The Contractor must notify members of the Underground Service Alert (USA) 48 hours in advance of performing excavation work by calling the toll free number (1-800-642-2444) or 811. 2. Contractor must field verify all existing highway and utility facilities' locations and depths. It will be the Contractor's sole responsibility to locate and protect all existing highway and utility facilities, so that no damage occurs to them during the performance of this contract. The Contractor must cooperate with other contractors and utility companies installing new structures, utilities and services to the project. Payment for this work will be considered to be included in the various items of work. 3. The Contractor must obtain all permits and licenses required for the construction and completion of the project, and must perform all work in accordance with the requirements and conditions of all permits and approvals applicable to the project. 4. The Contractor must exercise due caution and carefully preserve survey bench marks, control points, reference points and all survey stakes, and must bear all expenses for replacement and/or errors caused by their unnecessary loss or disturbance, 5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain in place unless otherwise noted. 6. Hand grinding of existing roadway surface will be required at locations where existing utility valves, maintenance holes, gutter pan under roadway surface and the lip of the gutter at drain inlet locations are within the construction work limits. Payment for hand grinding roadway surface is considered as included in the Planing Roadway Surfacing item of work. 7. Existing highway and utility facilities must remain in place unless otherwise noted. 8. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair and placement of corrugated steel pipes has been in place for a minimum of 18 hours. 9. 0.17' HMA Overlay must be placed in one (1) layer with Geosynthetic Pavement Interlayer (Paving Fabric) placed on the existing or prepared surface. 10. Placement of HMA Driveways must not begin until the HMA used for Mainline Paving has been completed. 11. Existing drainage facilities must remain in place unless otherwise noted. The Contractor must conduct earthwork operations so as not to modify or obstruct the existing drainage. All Earthwork, except for final shoulder grading, must be completed prior to placing the final layer of HMA Pavement. No Shoulder Backing material will be deposited or drifted onto the final layer of HMA surfacing. Where the shoulder placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted layers. 13. Shoulder Backing material at all intersection radii must receive Penetration Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be confined to intersection radii plus fifty feet (50') in each direction or to the intersection conform. 14. The Contractor will be responsible for adequately scheduling inspection and testing of all facilities constructed under this contract with a minimum 24 hour prior notice to the County. All testing must conform to the Standard Specifications.
