GENERAL NOTES: <br />1. Prior to commencing any work, it will be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor must notify members of the Underground Service Alert (USA) 48 hours <br />in advance of performing excavation work by calling the toll free number <br />(1-800-642-2444) or 811. <br />2. Contractor must field verify all existing highway and utility facilities' locations and <br />depths. It will be the Contractor's sole responsibility to locate and protect all <br />existing highway and utility facilities, so that no damage occurs to them during the <br />performance of this contract. The Contractor must cooperate with other contractors <br />and utility companies installing new structures, utilities and services to the project. <br />Payment for this work will be considered to be included in the various items of <br />work. <br />3. The Contractor must obtain all permits and licenses required for the construction <br />and completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />4. The Contractor must exercise due caution and carefully preserve survey bench <br />marks, control points, reference points and all survey stakes, and must bear all <br />expenses for replacement and/or errors caused by their unnecessary loss or <br />disturbance, <br />5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />6. Hand grinding of existing roadway surface will be required at locations where <br />existing utility valves, maintenance holes, gutter pan under roadway surface and <br />the lip of the gutter at drain inlet locations are within the construction work limits. <br />Payment for hand grinding roadway surface is considered as included in the <br />Planing Roadway Surfacing item of work. <br />7. Existing highway and utility facilities must remain in place unless otherwise noted. <br />8. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair <br />and placement of corrugated steel pipes has been in place for a minimum of 18 <br />hours. <br />9. 0.17' HMA Overlay must be placed in one (1) layer with Geosynthetic Pavement <br />Interlayer (Paving Fabric) placed on the existing or prepared surface. <br />10. Placement of HMA Driveways must not begin until the HMA used for Mainline Paving <br />has been completed. <br />11. Existing drainage facilities must remain in place unless otherwise noted. All Earthwork, except for final shoulder grading, must be completed prior to placing <br />the final layer of HMA Pavement. No Shoulder Backing material will be deposited or <br />drifted onto the final layer of HMA surfacing. Where the shoulder placement <br />exceeds 0.50' in thickness, the Shoulder Backing must be placed in two (2) or <br />more fully compacted layers. <br />13. Shoulder Backing material at all intersection radii must receive Penetration <br />Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work <br />must be confined to intersection radii plus fifty feet (50') in each direction or to <br />the intersection conform. <br />14. The Contractor will be responsible for adequately scheduling inspection and testing <br />of all facilities constructed under this contract with a minimum 24 hour prior <br />notice to the County. All testing must conform to the Standard Specifications.
