M <br />m <br />r - <br />m <br />z <br />0 <br />1. <br />2. <br />3. <br />4. <br />5. <br />A <br />7. <br />8. <br />9. <br />GENERAL NOTES: <br />0.25' HMA overlay must be placed in two (2) layers of approximately equal thickness <br />with Geosynthetic Pavement interlayer (Paving Fabric) placed on the existing or <br />prepared surfaced. <br />Placement of HMA Overlay must not begin until the HMA used in Pavement Repair has <br />been in place for a minimum of 18 hours. <br />Existing highway and utility facilities must remain in place unless otherwise noted. <br />Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain <br />in place unless otherwise noted. <br />Existing drainage facilities must remain in place unless otherwise noted. The Contractor <br />must conduct earthwork operations so as not to modify or obstruct the existing <br />drainage. <br />All Earthwork, except for final shoulder grading, must be completed prior to placing the <br />final layer of HMA pavement. No Shoulder Backing material will be deposited or drifted <br />onto the final layer of HMA surfacing. Where the shoulder placement exceeds 0.50' in <br />thickness, the Shoulder Backing must be placed in two (2) or more fully compacted <br />layers. <br />Shoulder Backing material at all intersection radii must receive Penetration Treatment at <br />0.25 gallon/square yard. Limits of the Penetration Treatment work must be confined to <br />intersection radii plus fifty feet (50') in each direction or to the intersection conform <br />at 4' width. <br />The Contractor will be responsible for adequately scheduling inspection and testing of <br />all facilities constructed under this contract with a minimum 24-hour notice to the <br />County. All testing must conform to the Standard Specifications. All re -testing will be <br />paid by the Contractor. <br />Hand grinding of roadway surfacing will be required at locations where existing highway <br />and utility facilities are within the Cold Plane Pavement and Pavement Repair limits. <br />Payment for hand grinding will be considered as included in the contract unit price Const <br />paid for Cold Plane Pavement. The Contractor must obtain an Encroachment Permit from the City of Lodi for <br />placement for performing traffic control or placing traffic control devices in their <br />jurisdiction. Payment for this will be considered to be included in the various items of <br />work involved. <br />11. Prior to cold planing pavement, the Contractor must remove all existing yellow <br />thermoplastic traffic striping within project limits in accordance with Section 84, <br />"Markings", of the Standard Specifications and special provisions. Note that <br />thermoplastic striping exists in the planing area from Sta 240+55 to Sta 240+75. <br />12. The Contractor must obtain all permits and licenses required for the construction and <br />completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />13. The Contractor must exercise due caution and carefully preserve survey bench marks, <br />control points, reference points and all survey stakes, and must bear all expenses for <br />replacement and/or errors caused by their unnecessary loss or disturbance. <br />14. Prior to commencing any work, it will be the Contractor's responsibility to have each <br />utility company locate in the field their main service lines and laterals. The Contractor <br />must notify members of the Underground Service Alert (USA) 48 hours in advance of <br />performing excavation work by calling the toll free number (1-800-642-2444) or 811. <br />15. The Contractor must field verify all existing highway and utility facilities' locations and <br />depths. It will be the Contractor's sole responsibility to locate and protect all existing <br />highway and utility facilities, so that no damage results to them during the <br />performance of this contract. The Contactor will be required to cooperate with other <br />contractors and utility companies installing new structures, utilities and services to the <br />project.
