V) Q D W m X r - M z O GENERAL NOTES: 1. 0.25' HMA Overlay must be placed in two (2) layers of approximately equal thickness. 2. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair has been in place for a minimum of 18 hours. 3. Placement of HMA Driveways must not begin until the HMA used for Mainline Paving has been completed. 4. Existing highway and utility facilities must remain in place unless otherwise noted. 5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain in place unless otherwise noted. 6. Existing drainage facilities must remain in place unless otherwise noted. The Contractor must conduct earthwork operations so as not to modify or obstruct the existing drainage. 7. Hand grinding of roadway surfacing will be required at locations where existing highway and utility facilities are within the Cold Plane Pavement and Pavement Repair limits. Payment for hand grinding will be considered as included in the contract unit price paid for Cold Plane Pavement. 8. All Earthwork, except for final shoulder grading, must be completed prior to placing the final layer of HMA pavement. No Shoulder Backing material will be deposited or drifted onto the final layer of HMA surfacing. Where the shoulder placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted layers. 9. Shoulder Backing material at all intersection radii must receive Penetration Treatment at 0.25 gallon/square yard. The Contractor will be responsible for adequately scheduling inspection and testing of all facilities constructed under this contract with a minimum 24-hour prior notice to the County. All testing must conform to the Standard Specifications. All re—testing will be paid by the Contractor. 11. The Contractor must obtain an Encroachment Permit from the City of Ripon for placement for performing traffic control or placing traffic control devices in their jurisdiction. Payment for this will be considered to be included in the various items of work involved. 12. The Contractor must exercise due caution and carefully preserve survey bench marks, control points, reference points and all survey stakes, and must bear all expenses for replacement and/or errors caused by their unnecessary loss or disturbance. 13. Prior to commencing any work, it will be the Contractor's responsibility to have each utility company locate in the field their main service lines and laterals. The Contractor must notify members of the Underground Service Alert (U.S.A.) 48 hours in advance of performing excavation work by calling the toll free number (1-800-642-2444) or 811. 14. The Contractor must field verify all existing highway and utility facilities' locations and depths. It will be the Contractor's sole responsibility to locate and protect all existing highway and utility facilities, so that no damage results to them during the performance of this contract. The Contractor will be required to cooperate with other contractors and utility companies installing new structures, utilities and services to the project.
