GENERAL NOTES: <br />1. 0.17' HMA Overlay must be placed in one (1) layer. <br />2. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair has been in place for a minimum of 18 hours. <br />3. Placement of HMA Driveways must not begin until the HMA used for Mainline Paving has been completed. <br />4. Existing highway and utility facilities must remain in place unless otherwise noted. <br />5. Existing trees, shrubs, fences, mailboxes and other frontage improvements must remain in place unless otherwise noted. <br />6. Existing drainage facilities must remain in place unless otherwise noted. The Contractor must conduct earthwork operations so as not to modify or obstruct the existing drainage. <br />7. All Earthwork, except for final shoulder grading, must be completed prior to placing the final layer of HMA Pavement. No Shoulder Backing material will be deposited or drifted onto <br />the final layer of HMA surfacing. Where the shoulder placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in two (2) or more fully compacted Iayers.Hand <br />grinding of roadway surfacing will be required at locations where existing highway and utility facilities are within the Cold Plane Pavement and Pavement Repair limits. Payment for <br />hand grinding will be considered as included in the contract unit price paid for Cold Plane Pavement. <br />8. Shoulder Backing material at all intersection radii must receive Penetration Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment work must be confined to <br />intersection radii plus fifty feet (50') in each direction or to the intersection conform. <br />9. The Contractor will be responsible for adequately scheduling inspection and testing of all facilities constructed under this contract with a minimum 24—hour notice to the County. All <br />testing must conform to the Standard Specifications. All re—testing will be paid by the Contractor. <br />10.The Contractor must exercise due to caution and carefully preserve survey bench marks, control points, reference points and all survey stakes, and must bear all expenses for <br />replacement and/or errors caused by their unnecessary loss or disturbance. <br />ll.Prior to commencing any work, it will be the Contractor's responsibility to have each utility company locate in the field their main service lines and laterals. The Contractor must <br />notify members of the Underground Service Agency (USA) 48 hours in advance of performing excavation work by calling the toll free number (1-800-642-2444) or 81 1. <br />12. The Contractor must field verify all existing highway and utility facilities' locations and depths. It will be the Contractor's sole responsibility to locate and protect all existing highway <br />and utility facilities, so that no damage results to them during the performance of this contract. The Contractor will be required to cooperate with other contractors and utility <br />companies installing new structures, utilities and services to the project. Payment for this work will be considered to be included in the various items of work involved. <br />13.Removal of existing HMA Speed Cushion on Bird Avenue to be paid under the Cold Plane Pavement item of work.
