<br />1. Existing highway and utility facilities must remain in place unless otherwise noted.
<br />2.All existing trees, shrubs, fences, concrete walks and other frontage improvements are to remain in place unless otherwise noted. Payment for removal of trees, shrubs and fences
<br />noted in plans must be considered as paid under Clearing and Grubbing.
<br />3.The Contractor will be responsible for adequately scheduling inspection and testing of all facilities constructed under this contract with a minimum 24—hour notice to the County.
<br />All testing must conform to the Standard Specification. All re—testing will be paid by the Contractor.
<br />4.The Contractor must obtain all permits and licenses required for the construction and completion of the project, and must perform all work in accordance with the requirements
<br />and conditions of all permits and approvals applicable to the project.
<br />S.The Contractor must exercise due caution and carefully preserve survey bench marks, control points, reference points and all survey stakes, and shall bear all expenses for
<br />replacement and/or errors caused by their unnecessary loss or disturbance.
<br />6.All improvements within the Minor Concrete (Curb, Gutter and Sidewalk) work limits must be saw cut or removed to the limits shown on the Plans. Payment for this will be
<br />considered as paid for under the Remove Concrete item of work.
<br />7.Prior to commencing any work, it will be the Contractor's responsibility to have each utility company locate in the field their main service lines and laterals. The Contractor must
<br />notify members of the Underground Services Alert (U.S.A.) 48 hours in advance of performing excavation work by calling the toll—free number (1-800-642-2444 or 81 1)
<br />B.The Contractor must field verify all existing highway and utilities facilities' locations and depths. It will be the Contractors' sole responsibility to locate and protect all existing
<br />highway and utility facilities, so that no damage results to them during the performance of this contract. The Contractor will be required to cooperate with other contractors and
<br />utility companies installing new structures, utilities and services to the project. Payment for this work will be considered as included in the various items of work.
<br />9.The Contractor must place temporary ramps to affected residents' driveways at the end of each working day if a drop off greater than 0.15' exists. Ramp materials must be
<br />consistent with that of the roadbed (or roadway) surface that it is to be placed upon. Payment for this work will be considered as included in the various items of work.

<br />10. All Part Circle Culverts or Steel Plates at driveways must be removed and returned to residents if requested or Contractor will dispose. Payment for these
<br />will be considered as
<br />included in the Clearing and Grubbing item of work.
<br />1 1. Payment for removing traffic stripes and pavement markings will be considered as paid under Roadway Excavation.
<br />12. Payment for constructing new concrete Driveways & Walkways will be considered as paid under Minor Concrete (Misc. Areas).
<br />13. Storm Drain Inlets must be protected from runoff sediment and chemical pollutant in a manner consistent with the the requirements of the National Pollution
<br />Discharge
<br />Elimination System (NPDES) general construction permit and per special provisions.
<br />14. All exposed roadway subgrade must be covered with Subgrade Enhancement Geotextile (Class 132) and a minimum of 6" of Class II Aggregate Base at the
<br />end of each working
<br />day.
<br />15. All new Curb, Gutter and Sidewalk must match existing conform to the existing flowline elevations with a straight grade between conforms, unless otherwise
<br />noted.
<br />16.Install Expansion Joints at beginning and end of new Curb, Gutter and Sidewalk and between new sidewalks and existing driveways and walkways.
<br />17. All Earthwork must be completed prior to placing the final layer of HMA Pavement. No Earthwork material will be deposited or drifted on the final layer of
<br />HMA surfacing.
<br />18. Payment for Earthwork and HMA Miscellaneous Areas when necessary must be considered as paid under Minor Concrete (Curb, Gutter and Sidewalk).
<br />19.0.30' HMA Overlay must be placed in two (2) layers of approximately equal thickness.
<br />20.Existing drainage facilities, fences, water valves, frames and covers, water meters, trees, shrubs, curb, gutter and sidewalk, etc. must remain in place unless otherwise noted on
<br />the Plans.
