GENERAL NOTES: <br />1. Prior to commencing any work, it will be the Contractor's responsibility to <br />have each utility company locate in the field their main service lines and <br />laterals. The Contractor must notify members of the Underground Service <br />Alert (U.S.A.) 48 hours in advance of performing excavation work by calling <br />the toll free number (1-800-227-2600). <br />2. The Contractor must field verify all existing highway and utility facilities' <br />locations and depths. It will be the Contractor's sole responsibility to locate <br />and protect all existing highway and utility facilities, so that no damage <br />results to them during the performance of this contract. The Contractor <br />will be required to cooperate with other contractors and utility companies <br />installing new structures, utilities and services to the project. Payment for <br />this work will be considered to be included in the various items of work. <br />3. The Contractor must obtain an Encroachment Permit from the State of <br />California for traffic control or any work within the State's jurisdiction. <br />Payment for this will be considered to be included in the various items of <br />work. <br />4. Placement of Hot Mix Asphalt Overlay must not begin until the Hot Mix <br />Asphalt used in Pavement Repair has been in place for a minimum of 18 <br />hours. <br />5. Placement of Hot Mix Asphalt Driveways must not begin until the Hot Mix <br />Asphalt used for Mainline Paving has been completed. <br />6. Existing highway and utility facilities, trees, shrubs, fences, mailboxes and <br />other frontage improvements must remain in place unless otherwise noted. <br />7. Existing drainage facilities (concrete headwall, culvert, etc.) must remain in <br />place unless otherwise noted. The Contractor must conduct earthwork <br />operations so as not to modify or obstruct the existing drainage. <br />8. All Earthwork, except for final shoulder grading, must be completed prior to <br />placing the final layer of Hot Mix Asphalt Pavement. No Shoulder Backing <br />material will be deposited or drifted on the final layer of Hot Mix Asphalt <br />surfacing. Shoulder Backing material at all intersection radii must receive Penetration <br />Treatment at 0.25 gallon/square yard. Limits of the Penetration Treatment <br />work must be confined to intersection radii plus fifty feet (50') in each <br />direction or to the intersection conform. <br />10. The Contractor will be responsible for adequately scheduling inspection and <br />testing of all facilities constructed under this contract with a minimum 24 <br />hour notice to the County. All testing must conform to the Standard <br />.D <br />Specifications.
