0 <br />r� <br />m <br />z <br />0 <br />GENERAL NOTES: <br />1. The HMA for Mainline Paving and Pavement Repair must be 3/�;- inch Type A and <br />must be "i /2 inch Type A for Driveways. <br />2. The asphalt binder for the HMA Type A must be PG 70-10 for Mainline Paving <br />and Pavement Repairs and must be PG 64-10 for Driveways. <br />5. 0.17' HMA Overlay must be placed in one (1) layer. <br />6. 0.25' HMA overlay must be placed in two (2) layers of approximately equal <br />thickness on the existing surface. <br />7. Placement of HMA Overlay must not begin until the HMA used in Pavement Repair <br />has been in place for a minimum of 18 hours. <br />8. Existing highway and utility facilities must remain in place unless otherwise noted, <br />9. Existing trees, shrubs, fences, mailboxes and other frontage improvements must <br />remain in place unless otherwise noted. <br />10. Existing drainage facilities must remain in place unless otherwise noted. The <br />Contractor rr;ust conduct earthwork operations so as not to modify or obstruct the <br />exi��ting drainage. <br />11. All Earthwork, except for final shoulder grading, must be completed prior to <br />placing the final layer of HMA pavement. No Shoulder Backing material will be <br />dep:.�sited or drifted onto the final layer of HMA surfacing. Where the shoulder <br />placement exceeds 0.50' in thickness, the Shoulder Backing must be placed in two <br />(2) or r�;ore fully compacted layers. <br />12. Shoulder Backing material at all intersection radii must receive P�:r,etration <br />Treatment at 0.25 gallon/square yard. Limits of the Per;etration Treatment work <br />rnus�� be confined to the inside of intersection radii plus fifty feet (50') in each <br />dirF;ction or to the intersection conform at 6' width, <br />13. The Contractor will be responsible for adequately scheduling inspection and testing <br />or all facilities constructed under this contract with a minimum 24—hour notice to <br />the County. All testing must conform to the Standard Specifications. All re —testing <br />will be paid by the Contractor. <br />14, Hand grinding of roadway surfacing will be required at locations where existing <br />highway and utility facilities are within the Cold Plane Pavement and Pavement <br />Repair limits. Payment for hand grinding will be considered os included in the <br />contract unit price paid for Cold Plane Pavement, <br />15, The Contractor must obtain an Encroachment Permit from the State Department of <br />Transportation, City of Lathrop, and City of Tracy for placement for performing <br />traffic control or placing traffic control devices in their jurisdiction. Payment for <br />this wil9 be considered t� be included in the various items of work involved, <br />16, The Contractor must obtain all permits and licenses required for the construction <br />and completion of the project, and must perform all work in accordance with the <br />requirements and conditions of all permits and approvals applicable to the project. <br />17, The Ccr�tractor must exercise due ;caution and carefully preserve survey be;�ch <br />marks, control points, reference points and all survey stakes, and must bear a' <br />expenses for replacement and/or errors caused by their unnecessary loss or <br />disturbance. <br />18, Prior to commencing any work, it will be the Contractor's responsibility to have <br />each utility company locate in the field their main service lines and laterals. The <br />Contractor must notify members of the Underground Service Alert (USA) 48 hours <br />in advance of performing excavation work by calling the toll free number <br />(1-800-642-2444) or 81 1. <br />19, The Contractor must field verify all existing highway and utility facilities' locations <br />and depths. It will be the Contractor's sole responsibility to locate and protect all <br />existing highway and utility facilities, so that no damage results to them during <br />the performance of this contract. The Contactor will be required to cooperate with <br />other contractors and utility companies installing new structures, utilities and <br />services to the project, Payment for this work will be considered to be included in <br />the various items of w rk involved. <br />20. Roadways, or segments 'hereof, that are less than 20 feet in paved width may be <br />temporarily closed during construction activities. Schedule of closureis) must be <br />approved by the Engineer, Contractor must provide a two weeks advance notice to <br />all affected residents, services, school, etc. of the scheduled closure° Content of <br />public notification must 5e approved by the Engineer. <br />21.
