There was up to 2 feet of fast flowing water in the slough partially <br />below Spans 2 and 4 and below Span 3 during this investigation. Bents 3 and 4 were submerged and Bent 3 was probed from the <br />waterline using a 30 foot pole. Due to the high velocity flow wading and probing at Bent 4 was not an option. The last complete routine <br />inspection of Bent 4 was completed on 03/08/2021. All remaining visible elements were fully inspected. A complete routine inspection was <br />performed. <br />NUMBERING CONVENTION <br />The support numbering system used on the as-built plans for this structure is reversed from the standard numbering convention used by <br />Structure Maintenance & Investigations. For this structure, the southern support is Abutment 1, and the northern support is Abutment 4. <br />This report and all subsequent reports will follow the SM&I numbering convention. <br />MISCELLANEOUS <br />A new routine map view photo was added during this investigation. See Photo 1. <br />DECK AND ROADWAY <br />There are transverse and longitudinal cracks up to 0.25 inches wide, widely spaced, in the AC approaches at both ends of the bridge. In <br />addition, there is a pothole up to 6 inches in diameter x 2 inches deep in the northbound lane at Abutment 1. See Photo 2. <br />WATERWAY <br />A Caltrans Hydraulics Report dated 09/10/2019 determined this structures appropriate NBI Item 113 rating be updated from 3 to 7, <br />Countermeasures have been installed to mitigate an existing problem with scour and to reduce the risk of bridge failure during a flood <br />event. Instructions contained in a plan of action have been implemented to reduce the risk to users from a bridge failure during or <br />immediately after a flood event. <br />A Bridge Scour Evaluation - Plan of Action dated 10/31/2005 is on file within the SM&I electronic database.
