Bent 2 was submerged in 8
inches of clear water. The deck and superstructure were inspected by walking on and about the bridge. The Substructure elements were
accessed and inspected by wading and probing. A complete routine bridge inspection was performed.
MISCELLANEOUS
Photographs were taken during this inspection to replace the: Routine-Roadway View, Routine-Elevation View, and the Routine-
Underside View. The Routine-Map View was also updated. Refer to Photos 1 through 4.
DECK AND ROADWAY
There is a small raised curb on the traffic side of the sidewalk that is broken off near abutment 1 right.
WATERWAY
A channel cross section was measured at the time of this inspection and compared to the channel cross section taken 01/16/2014. A
comparison shows the channel has risen 6 inches at Bent 2 and up to 18 inches at the toe of the slopes.
INSPECTION COMMENTARY
TYPE OF MEMBRANE
TYPE OF DECK PROTECTION
DECK STRUCTURE TYPE
TYPE OF WEARING SURFACE
WEARING SURFACE / PROTECTIVE SYSTEM
95AVERAGE DAILY TRAFFIC
2013 YEAR OF ADT
2 KMBYPASS, DETOUR LENGTH
5 %(109) TRUCK ADT %
273FUTURE ADT
2042YEAR OF FUTURE ADT
HISTORICAL SIGNIFICANCE 4: HIST SIGN NOT DETERMI
0-NONE
0-NONE
0-NONE
1-CIP CONCRETE
DECK STRUCTURE INFORMATION
11.9 M
9.8 M
DECK AND ROADWAY
LANES 2
25
0-NOT ON NHS
PARALLEL STRUCTURE
TEMPORARY STRUCTURE
DEFENSE HIGHWAY
FUNCTIONAL CLASS
SPEED
NATIONAL HIGHWAY SYSTEM
TOLL
DIRECTION OF TRAFFIC
DESIGNATED NATIONAL NETWORK
TYPE OF SERVICE
FEDERAL LANDS HWY
6 EQUAL MIN CRITERIAAPPR ROADWAY ALIGN
8 DESIRABLE CRITERIADECK GEOMETRY
6.1 MAPPROACH RDWY WIDTH
BRIDGE MEDIAN
LEFT 0.0 M RIGHT 1.5 MCURB OR SIDEWALK
TOTAL WIDTH
NET WIDTH
9.8 MINVENTORY ROUTE TOTAL HORIZ CLEAR
99.99 MINVENTORY ROUTE MIN VERT CLEAR
3-ON FREE ROAD
09-LOCAL RURAL
0-NOT STRAHNET
N-NONE EXISTS
2-2 WAY
0-NOT APPLICABLE
0-NOT ON NET
0 NO MEDIAN
5-HIGHWAY-PEDESTRIAN
DECK ROADWAY/OPERATIONAL INFORMATIONDECK GEOMETRY
1.0 ft br, 32.0 ft, 5.0 ft sw, 1.0 ft br
DECK CROSS SECTION
LRS INVENTORY RTE & SUBRTE
BASE HIGHWAY NETWORK 0-NOT ON NET
- NOT APPLICABLE FOR THIS BRIDGE.
0 inchesOVERLAY THICKNESS (inches)
N/A
(51)
(52)
(50)
(32)
(33)
(107)
(108)
A)
B)
C)
(29)
(30)
(19)
(114)
(115)
(37)
(42a)
(12)
(13)
(104)
(26)
(100)
(101)
(102)
(10)
(47)
(68)
(72)
(105)
(110)
(20)
(28a)
(103)
CONDITION INFORMATION
16.5 MLENGTH (49)
Element DescriptionElem
No.
Env Total
Qty
Units
CS 1 CS 2 CS 3 CS 4
Qty in each Condition State
38 Slab-RC
Cracking (RC and Other)
2
2
190
190
sq.m
each
0
0
190
190
0
0
0
0
DECK ELEMENT INSPECTION RATINGS AND NOTES
Defect/
Prot
1130
Defect
(38-1130) Cracking (RC and Other)
(58) 5DECK RATING =
SPECIAL INSPECTION INFORMATION
