The creek was dry during this investigation. A complete inspection of <br />the visible elements was performed. <br />MISCELLANEOUS <br />A new routine map view photo was added to this report. See Photo 1. <br />DECK AND ROADWAY <br />There is a 3 foot long x 18 inch wide x 2 inch deep depression of broken concrete in the left approach sidewalk at Abutment 4. See Photo <br />2. In addition, 2 feet south of the depression there is a 3 foot long x 3 inch high void under the sidewalk. See Photo 3. There are similar <br />voids under the approach sidewalks at both abutments on the right side of the bridge. The approach sidewalks are undermined up to 12 <br />inches at all locations. <br />Type P object makers were present at the right side of Abutment 1 and the left side of Abutment 4 at the ends of the wingwalls. <br />SUBSTRUCTURE <br />New RSP has been placed at both slope embankments. In addition, a concrete slurry mix was poured over the RSP primarily below <br />Spans 1 and 3.
