Printed on:Friday 12/13/2024 03:28 PM 29C0284/AAAK/144221Pageof3 <br />Department of Transportation - Division of Maintenance Structure Maintenance & Investigations <br />6 <br />UNDERWATER INVESTIGATION DETAILS - NOT APPLICABLE FOR THIS BRIDGE. <br />STEEL INVESTIGATION DETAILS <br />SCOPE AND ACCESS <br />All elements were accessible by foot on and around the structure. There was 12 inches of clear water in the creek in Spans 2 and 3 <br />during this investigation. Pier 3 was submerged and inspected with the aid of a probe. All visible elements were fully inspected. A <br />complete routine inspection was performed. <br />NUMBERING CONVENTION <br />The alignment of the structure is from northwest to southeast making the location of Abutment 1 unclear. For this structure, the northwest <br />abutment is Abutment 1 and the southeast abutment is Abutment 4. <br />MISCELLANEOUS <br />A routine map view photo was added during this investigation. See Photo 1. <br />DECK AND ROADWAY <br />There is a 2 inch gap at the paving notch at Abutment 4. See Photo 2. <br />There is heavy vegetation growing over the right bridge rail in Span 1. See Photo 3. <br />WATERWAY <br />The embankment slope at Abutment 1 has eroded to the face of the abutment. (See Photo 3 dated 09/15/2020). This condition has been <br />previously reported and no significant changes were noted. <br />INSPECTION COMMENTARY <br />- NOT APPLICABLE FOR THIS BRIDGE. <br />CONDITION INFORMATION <br />SPECIAL INSPECTION INFORMATION
