The creek was dry during this investigation. All visible elements were <br />fully inspected. A complete routine inspection was performed. <br />NUMBERING CONVENTION <br />The support numbering system used on the as-built plans for this structure is reversed from the standard numbering convention used by <br />Structure Maintenance & Investigations. For this structure, the southern support is Abutment 1, and the northern support is Abutment 2. <br />This report and all subsequent reports will follow the SM&I numbering convention. <br />MISCELLANEOUS <br />A new routine map view photo was added during this investigation. See Photo 1. <br />DECK AND ROADWAY <br />The previously reported pothole in the AC approach roadway located in the northbound lane at Abutment 1 has been filled with AC, <br />however, there is still a depression up to 1 inch across the full width of the lane. (See Photo 2). The previously reported pothole in the AC <br />approach roadway located in the southbound lane at Abutment 2 has been filled with AC. See Photo 3. <br />WATERWAY <br />The 12/05/2006 Hydraulic Report revised NBI Item 113 Code, Vulnerability to Scour, from 3 to 7 after scour mitigation work was <br />completed by the County. Broken concrete rip-rap filling the hole at Abutment 2 underneath the bridge and protecting the upstream <br />Abutment 1 and 2 wingwall slopes and portions of the approach road embankments were observed. The left Abutment 2 wingwall slope <br />and the embankment was also protected with gabion baskets and broken concrete. The 12/05/2006 report compared channel cross- <br />sections taken since 1995 and determined the channel has been relatively stable. It was noted that the channel has a wide flow area <br />upstream of the bridge, which could result in contraction flow during the high flood events. Based on the field investigation in 2006 and <br />the scour countermeasures that had been installed, the 12/05/2006 Hydraulic Report stated this structure is considered not to be scour <br />critical at this time. However, it was recommended that the County continue to monitor the scour countermeasure and its performance <br />periodically or during the high flood events as indicated on the 11/02/2005 Bridge Scour Evaluation - Plan of Action form. <br />The 10/17/2002 Bridge Inspection Report indicated that the gabion baskets had pulled away from the left Abutment 2 wingwall. This <br />condition still exists and the gap between the wingwall and the gabions are filled in with loose pieces of broken concrete. See Photos 2 <br />and 3 dated 10/08/2016. This condition has not changed when compared to previous photos.
