(See Photo 3 dated <br />09/14/2020). <br />*Girder 1 on the right side there is a partially filled epoxy patch spall up to 6 inches in diameter that is sound. There is a <br />corresponding epoxy injected vertical crack at this location. (See Photo 5 dated 09/21/2022). <br />*Girder 4 on the left side there is a spall up to 14 inches wide x 20 inches tall x 4 inches deep in the diaphragm face. (See Photo 7 <br />dated 09/19/2018). <br />*Girder 4 on the right side there is a spall up to 24 inches in diameter x 4 inches deep with one exposed rebar at the diaphragm face. <br />(See Photo 6 dated 09/21/2022). <br />Abutment 8: <br />*Girder 1 on the right there is a spall up to 12 inches in diameter x 2 inches deep in the diaphragm face. (See Photo 7 dated <br />09/21/2022). <br />*Girder 3 on the right there is a spall up to 2 feet high x 12 inches wide x 1 inch deep in the diaphragm face. (See Photo 8 dated <br />09/21/2022). <br />*Girder 4 on the right there is a spall up to 3 inches diameter x 1 inch deep in the diaphragm face. <br />*At Girders 1, 2 and 4 on the left side there are delaminations up to 2 feet high x 12 inches wide in the diaphragm face. (See Photo 9 <br />dated 09/21/2022). <br />These conditions have been previously reported and no significant changes were noted. <br />This quantity includes monolithic wingwalls. <br />226 Pile -PS Conc. 2 1 ea. 1 0 0 0 <br />(226) Pile -PS Conc. <br />The pile element is included to indicate the presence of piles on this structure. The piles were not exposed for visual inspection.
