Waterloo Road (Site Plan enclosed). <br /> The well was installed in May 1996 under Encroachment Permit No. 96-014-NS (enclosed). <br /> Destruction of the well is requested by the San Joaquin County Environmental Health Department <br /> (EHD) as part of the environmental site closure_ The work will include destruction of monitoring <br /> wells MW-6 per EHD guidelines_ <br /> Also enclosed are a Traffic Control Plan and a check for $436.00 to cover the application and <br /> inspection fees.
