PS-1501620 July 22, 2015 <br /> SPECIAL CONDITIONS FOR <br /> CVIN — ESCALON BELLOTA RD. & LONE TREE RD. <br /> ENCROACHMENT PERMIT <br /> 1. The Escalon-Bellota portion of the project shall be completed prior to the start of the County's <br /> 2016 resurfacing project for Escalon-Bellota Road. <br /> 2. The conduit alignment shall be placed no closer than six (6) feet from the edge of pavement <br /> along Escalon-Bellota Road. <br /> 3. The conduit alignment shall be placed no closer than four (4) feet from the edge of pavement <br /> along Lone Tree Road. <br /> 4. Relative compaction of ninety (90) percent shall be achieved for plowed excavations. All grades <br /> and slopes will be restored to pre-job conditions, or County Standards, whichever is applicable. A <br /> smooth, compacted, drivable finish will be required for the finish grade of all shoulders. No <br /> broad-based vibratory compaction effort will be applied to the shoulder unless an area larger than <br /> 24" wide has been excavated. Surface compaction shall be achieved by pneumatic roller; any <br /> deviation must be approved by County Field Engineering. <br /> 5. Contractor shall restore shoulder areas in front of mailboxes with Class II Aggregate Base <br /> compacted to 95%. <br /> 6. The proposed route for the fiber optic cable crosses a state regulated channel (Lone Tree Creek) <br /> at three locations. These crossings will require permits from the State (Central Valley Flood <br /> Protection Board - CVFPB) and San Joaquin County Flood Management Division - SJCFMD <br /> (Endorsement Letter). <br /> CVIN shall apply directly to the Central Valley Flood Protection Board for the state permits, but <br /> first submit a copy of the application(s) directly to the San Joaquin County Flood Management <br /> Division for the associated Endorsement Letter(s). Fees for each crossing apply; per current San <br /> Joaquin County Public Works Fee Schedule. <br /> 7. No work shall occur within 30 feet of the top of bank or 30 feet from the landside toe of the levee <br /> (where a levee exists) until such time all required State and County permits are obtained as <br /> confirmed by the Flood Management Division of the San Joaquin County Dept. of Public Works <br /> (Phone: 209-468-3605). <br /> 8. Maintain traffic controls for all roads. Traffic control delays shall not exceed 15 minutes. <br /> Two-way traffic shall be maintained during non-working hours with excavated areas backfilled or <br /> plated. During working hours, two-way traffic control shall be maintained with one lane open and <br /> appropriate flaggers. <br /> 9. Access to all properties shall be maintained at all times except when work is occurring at the <br /> access point. Minimal delays will be allowed to provide access within the work zone area. <br /> Driveway access shall be fully restored at the end of each workday. Driveways disturbed by the <br /> contractor shall be replaced with in-kind or better materials. <br />
