The following is an itemized list of hazardous materials violations that have not been <br /> addressed for DIFFERENTIALS PLUS as of October22, 2018. <br /> Open violations from July 20, 2018 inspection <br /> Violation #2 - Failed to complete and electronically submit a hazardous materials business plan. <br /> The facility's business plan was not submitted electronically to the California Environmental Reporting System <br /> (CERS). As of January 1, 2013, all businesses were required to submit all business plans electronically. <br /> Immediately log into the CERS at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to <br /> the EHD for approval. <br /> Violation #3 - Failed to establish and implement a hazardous materials business plan. <br /> A Hazardous Materials Business Plan has not been established or implemented. A Hazardous Materials Business <br /> Plan shall be established and implemented for emergency response to a release or threatened release of a <br /> hazardous material when storing hazardous materials at or above the thresholds quantities of 55 gallons for <br /> materials that are liquids, 500 pounds for solids, or 200 cubic feet for compressed gas, at standard temperature and <br /> pressure. Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, <br /> enter the required information, and submit for review by the EHD. <br /> Violation #4 - Failed to complete and/or submit the Business Activities or Owner/Operator Identification <br /> page. <br /> The Business Owner/Operator Identification form was not submitted. This information must be current and <br /> submitted as part of the business plan. Immediately log into the California Environmental Reporting System (CERS) <br /> at http://cers.calepa.ca.gov/, enter the correct or updated information, and submit to the EHD for approval. <br /> Violation #5 - Failed to complete and/or submit hazardous materials inventory for all reportable materials on <br /> site. <br /> Hazardous Materials were found on site that have not been reported. Any material that meets or exceeds the <br /> reportable quantity shall be reported in the facility's business plan. Immediately log into the California Environmental <br /> Reporting System (CERS) at http://cers.calepa.ca.gov/, add the materials to the hazardous material inventory, and <br /> submit to the EHD for approval. <br /> Violation #6 - Failed to complete and/or submit a site map with all required content. <br /> A site map was not submitted as part of the business plan. A site map shall contain a north orientation, loading <br /> areas, internal roads, adjacent streets, storm and sewer drains, access and exit points, emergency shutoffs, <br /> evacuation staging areas, hazardous material handling and storage areas, and emergency response equipment. If <br /> a site map element is not applicable for your facility then list it on the map and label as"NA". Immediately log into <br /> the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the correct or updated <br /> information, and submit to the EHD for approval. <br /> Violation #7 - Failed to establish and/or submit emergency response procedures for a release or threatened <br /> release. <br /> The emergency response procedures were not uploaded to CERS website. A business plan shall include the <br /> following emergency response procedures for a release or threatened release of hazardous materials, including, but <br /> not limited to, the following: <br /> - immediate notification of local emergency personnel and the Environmental Health Department(EHD) <br /> - procedures for the mitigation of a release or threatened release to minimize any potential harm or damage to <br /> persons, property, or the environment <br /> -evacuation plans and procedures, including immediate notice, for the site <br /> Immediately log into the California Environmental Reporting System (CERS) at http://cers.calepa.ca.gov/, upload the <br /> correct or updated information, and submit to the EHD for approval. <br /> Page 1 of 2 <br />
