RECEIVED <br /> TRAINING DOCUMENTATION FORM SEC 1 2000 <br /> OF EMERGEWY S MES <br /> Business Name : CC v Z <br /> Business Address : r, S <br /> The employee training consists of reading the Hazardous Materials Management Plan <br /> (HMMP)with special attention to the Emergency Response Plan Procedures (ERPP) and. <br /> the location of all emergency equipment. Training also includes review of Material Safety <br /> f <br /> Data Sheets (MSDS)for chemicals stored on-site. The emergency equipment includes the <br /> First-Aid kit, fire extinguishers, absorbent (kitty litter), brooms and other clean-up tools. <br /> The location of the emergency equipment is graphically depicted on the site plan. The <br /> store operator is responsible for employee training and answering any questions about the <br /> MSDS, HMMP and ERPP. <br /> By signing this document, the employee is stating that he/she has read and understands <br /> the HMMP and ERPP. It is up to the employee to ask questions about the items he/she <br /> does not understand and it is up to the store operator to answer those questions.
