Original signed 1999 Hazardous Materials Management Plan/Inventory Certification Statement on behalf of Eurocorp Aviation signed by Perry Kotsoglou. 2 . Original signed 1998 Hazardous Materials Management Plan/Inventory Certification Statement on behalf of Eurocorp Aviation signed by Perry Kotsoglou. 3 . 10 page document signed by Perry Kotsoglou on 12/6/97 which includes a Declaration of Completeness and Accuracy which I have had Mr. Kotsoglou sign once again in blue ink as an original . This original of this document which purports to cover the calendar year beginning 1/1/98 and ending 1/1/99 was originally sent to the County in December of 1997 . Mr. Kotsoglou was of the understanding that this fulfilled his obligations during the 1998 calendar year. I understand that it is the Office ' s contention that this covered only the 1997 year. I do want you to understand that this was the result of an innocent misunderstanding in communication. When Mr. Kotsoglou received the 1998 form, he called the Office of Emergency Services and wanted to know why he had received the document because he had just filed his Plan. Mr. Kotsoglou was informed that they had received his Plan and could ignore the 1998 correspondence. When Mr. Kotsoglou was contacted concerning this matter, he turned everything over to his then attorney in Stockton, Dick Johnson. He instructed Mr. Johnson to take whatever steps were necessary to insure compliance .
