Box 31600 <br /> .Stockton, CA 95213 <br /> Subject: Hazardous Materials Management Plan - Delinquent Fees <br /> Dear Mr. Bywater: <br /> A bill for the 1993 Hazardous Materials Management Plan (HMMP) was mailed to your <br /> business. Your payment is now delinquent and a 10% late fee has been assessed. The amount <br /> owing for 1993 HMMP fees is $330.00. <br /> If a business is unable to pay the HMMP fee in one payment, they will be given the <br /> opportunity to make payments according to a set payment schedule. These payments will <br /> include a 10% late penalty. Please contact our office at 468-3969 to make arrangements. <br /> By law, all business licenses and building permits must be reviewed for compliance with the <br /> provisions of Chapter 6.95 of the California Health and Safety Code. If a business is <br /> delinquent in their annual fee, it could lead to delays or rejection of the license or permit <br /> process. <br /> Our office has sent notices that previous fees have not been paid. As of this date, the total <br /> amount now owing on this account is $960.00. Your payment should be made payable to the <br /> Office of Emergency Services.
